۴؍ اکتوبر ۱۹۸۰؁ء کو جناب ظفر احمد صدیقی وکیل، سکریٹری دینی تعلیم کونسل کا انتقال اپنے آبائی وطن راما بھاری تحصیل بسواں ضلع سیتاپور میں ۳۰:۷ بجے صبح کو طویل علالت کے بعد ہوگیا، دفتر دینی تعلیمی کونسل لکھنو میں وہ حضرت مولانا علی میاں صاحب کے ایماء پر ۵۹؁ء سے مقیم تھے، یہ ایک طرح سے ان کا وطن ثانی بن گیا تھا، اپنی وفات سے ۴۸ گھنٹے پہلے اس حال میں رخصت ہوئے تھے کہ انہیں ہوش نہیں تھا، بلڈ یوریا کی وجہ سے ایک ہفتہ سے غفلت تھی، احباب اور رفقاء نے اسی وقت یہ سمجھ لیا تھا کہ برسوں کا یہ تھکا ہوا مسافر اور ساتھی اب لکھنؤ واپس نہیں آئے گا، سیتاپور سے فون پر اطلاع ملی، وہ سب سے جدا ہوکر اﷲ کو پیارے ہوگئے، ان کی اہلیہ کا انتقال سال بھر پہلے ہوچکا تھا، اولاد کوئی نہیں تھی، بھتیجوں کو اولاد سمجھتے رہے، جن کو اپنی نگرانی میں تعلیم دلائی، ان کے حقیقی بھائی کا قیام رامابھاری میں ہے، جہاں ان کا خاندان صدیوں سے آباد ہے۔
ظفر صاحب نے تعلیم مسلم یونیورسٹی میں پائی، ان کا قیام میکڈانلڈ ہوسٹل میں تھا، تعلیم کے بعد سیتاپور میں وکالت شروع کی ان کا شمار وہاں کے کامیاب وکیلوں میں تھا، ۴۷؁ء سے پہلے مسلم لیگ سے وابستہ رہے، تبلیغی کاموں سے بھی شغف رکھا، لیکن جب قاضی محمد عدیل عباسی مرحوم نے ۵۹؁ء میں بستی میں بچوں کی تعلیم کے سلسلہ میں کنونشن کیا اور وہاں یہ فیصلہ ہوا کہ اس سلسلہ میں مستقل کام کی ضرورت ہے، تو حضرت مولانا علی میاں صاحب کے ایماء پر ظفر صاحب نے اپنی کامیاب وکالت چھوڑ دی اور دینی تعلیمی کونسل کے کاموں کے لئے اپنے کو وقف کردیا، پھر ان کو ایسی...

Factors Related to the Implementation of Evidenced-based Practice (EBP) of Nursing Staffing Levels in Hospitals: A Literature Review

The implementation of Evidenced-based practice (EBP) was one of the factors in increasing the quality of health services as an ideal problem-solving approach. The ability of nurses to appliance EBP was influenced by many factors, not only on individual nurses consisting by the support of unit leader and colleague. The purpose of this study was to determine the factors associated with the implementation of EBP at the care room level establishing on the support of coworkers and nursing managers. This writing used a literature review study, from several databases namely PubMed, ProQuest, Google Scholar, and ScienceDirect with a limitation of the last 5 years. 17 journals met the inclusion criteria and the results of a literature review study show: eight journals conclude that the support of colleagues and authority in changing practices affects the implementation of EBP, 13 journals concluded the role of managerial nurses affects the implementation of EBP consisting of leadership support and mentorship including the lack of knowledge and skills of nursing leaders regarding EBP. Data from the collected and identified literature study clearly showed that EBP knowledge and skills of nursing leaders related to EBP are important factors in increasing the implementation of EBP but co-worker factors can be a supporter and also an obstacle to the implementation of EBP if it is not well managed.

Immunohistochemical P21 Expression in Breast Cancer: Association With Patient and Tumour Characteristics

Introduction: Molecular characterisation of the breast cancer has allowed for targeted therapies that are improving outcomes. The gains achieved by these treatments have fuelled the search for other targets, including cyclin-dependent kinase inhibitors such as p21. p21 expression in breast cancer has been shown to be variably associated with clinical and pathological parameters in different populations. It is possible that either methodological or biological differences underlie these variations. Objective: The current study sought to determine the association of p21 expression and clinico-pathological parameters in breast cancer patients at the Aga Khan University Hospital in Nairobi, Kenya. Methods: Clinically annotated tissue samples were obtained in a consecutive fashion. Specimen were immunohistochemically stained for p21 expression with clinicopathological data extracted for corresponding cases. p21 expression was assessed against age, parity, menopausal status, body mass index, hormonal receptor status, tumour size and grade. Distribution of mean p21 score was assessed for normality and data were analysed using Kruskall-Wallis test, chi-square and Fisher's exact test at an alpha level of 0.05. They were presented in form of tables, charts and photomicrographs. Results: Ninety-eight cases were included in the study. Mean age was 47.55 years with mean parity 2.96 live births. A slight majority of patients were pre-menopausal (56.3% versus 42.5%). Average body-mass index was 29.69 kilograms per square metre. Majority of patients had invasive ductal carcinoma (87.6%) that was moderate-to-high grade (92.7%), 2 centimetres or larger (95%), with lymphovascular invasion (70.4%), and vi hormone-receptor positive. Mean expression of p21 at 1:100 dilution was approximately 20% was statistically significant with tumour size. Conclusion: The findings of the current study demonstrate a possible association of p21 expression with tumour size. The demonstration of a trend towards p21 loss with increasing body mass index may benefit from exploration with a larger dataset.
