20ستمبر1996ء کی کالی رات (آخری حصہ )

ہمارے گھر (70کلفٹن)کے باہر ہو نے والی شدید فائرنگ 45منٹس کے بعد رک چکی تھی ۔میرے پا پا (میر مرتضی بھٹو)کی اب تک کوئی خبر نہیں تھی ۔میں اپنی امی (غنویٰ بھٹو )کے ساتھ حالات کا جائزہ لینے باہر آئی اور پولیس والوں سے فائرنگ کی وجہ پو چھی تو انھوں نے جواب دیا کہ کچھ ڈاکوئوںسے ہمارا مقابلہ ہوا ہے ۔باہر خطر ہ ہے اس لیے آپ اندر رہیں پولیس والوں پہ یقین کر کے ہم واپس گھر آئے ۔

کچھ دیر بعد مجھے پتہ چلا کہ باہر ہو نے والی فائرنگ کسی اور پہ نہیں بلکہ میرے پا پا اور ان کے ساتھیوں پہ ہوئی ہے ۔فائرنگ بند ہو نے کے بعد 40سے 50منٹس تک میرے پا پا اور ان کے ساتھیوں کو جائے وقوعہ پر ہی تڑپتا چھوڑ دیا گیا ۔جس کا مقصد صرف یہی تھا کہ خون زیادہ بہنے کی وجہ سے تمام زخمی جن میں پا پا بھی شامل تھے اپنے آپ مر گئے ۔

اس پولیس کارروائی کا مقصد صرف ایک ہی تھا میرے پا پا کو قتل کر نا ۔یہ ایک منظم اور طے شدہ منصوبہ تھا ۔جس کی تیاری پولیس والوں کی جانب سے سے پچھلے کئی دنوں  سے جا ری تھی ۔

فائرنگ کے بعد پا پا کے کافی ساتھی موقع پر ہی شہید ہو گئے لیکن پا پا زخمی اور زندہ تھے ۔اگر پاپا کو جلدی سے طبی سہولیات ملتی تو وہ بچ سکتے تھے لیکن پولیس کو پا پا کے قتل کے احکا مات ملے ہو ئے تھے تو وہ کیوں پا پا کو طبی سہولیات دلواتے ۔فائرنگ بند ہونے کے تقریباََ 50منٹس بعد پا پا کو پولیس کی گاڑی میں...

Analysis of Factors Affecting Gillnet Fisherman Income 10 GT at Fish Auction Place, Indramayu Regency

The purpose of this study is to analyze the income level of gillnet fishermen <10 GT, to analyze the factors that affect the level of income, and to formulate strategies and solutions to increase income. This research was conducted at the Fish Auction Place (TPI) Pabean Udik, Pabean Udik Village, Indramayu Regency. This location was chosen because the Udik Customs Fish Auction Place (TPI) is dominated by fishermen using gillnet fishing gear who have vessels <10 GT. The method used is the census method with samples taken as a whole. The analysis used in this research is the Logistic Regression Analysis and the SWOT Analysis. Research results obtained by using IBM SPSS, namely for the Sig Value Variable in the Equation is 100%, this value is interpreted by 8 (factors) Independent variables can affect income only educational factors that do not affect income and for the results obtained from SWOT analysis namely to increase the income of gillnet fishermen <10 GT at TPI Pabean Udik, namely the development of technology and fisheries business scale, development of access to capital for fishermen, development of marketing access, development of facilities and infrastructure at TPI Pabean Udik, Development of diversification of fish management and environmentally friendly fisheries management.

Chronic Post Mastectomy Pain in Adult Female Patients at Aga Khan University Hospital Nairobi

Introduction: Breast cancer is a major cause of morbidity and mortality worldwide among women. The improvement in diagnostic and treatment strategies has led to a larger number of survivors who have breast cancer. However, there are complications attributed to breast cancer treatments, including chronic post mastectomy pain (CPMP). CPMP causes physical, social, emotional and functional disability. Once established, it is difficult to treat and negatively impact quality of life of the affected patients. Despite the big number of patients with breast cancer who undergo mastectomy at AKUHN, the burden of CPMP and factors associated with it remain unknown. Objectives: To determine the burden of CPMP and its associated risk factors in adult female patients at AKUHN. Study design: A descriptive cross-sectional study. Study setting: The AKUHN. Study population: Female breast cancer patients who underwent mastectomy at AKUHN between January 2008 and December 2017. Sampling process: Stratified sampling with subgroups based on year of surgery. Sample size: Assuming a CPMP prevalence of 10% and an attrition rate of 10% in the study population, a sample of 370 patients would be required to detect an odds ratio of developing CPMP of 2.5 with a p-value< 0.05. Statistical analysis: By the use of SPSS version20. The study outcome: A total of 480 patients’ contacts were successful, of which 49 declined the interview, 22 were deceased, while 409 patients were alive and responded to the study interview. The prevalence of PMCP was 14.4% with 9.3% who also had arm pain. The severity of pain was distributed as mild, moderate and severe at 55.9% (33), 40.7% (24) and 3.4% (2) respectively. The only predictor of CPMP which was statistically significant in this current study was age while the rest were not. Conclusion: The prevalence of CPMP in the current study in our institution is relatively low compared to other studies. In the study, the only associated factor with CPMP that is statistically significant is age; which corresponds with the studies reviewed. Recommendation: We recommend a closer follow up of patients under the age of fifty years as they are more prone to develop CPMP. We also recommend a multicenter study which will give a larger sample size with a population of diverse demographic profiles; subjected to different types and techniques of breast surgeries, different surgical set-ups; different post-operative care; and different post-operative pain management for a more conclusive outcome.
