This study is an attempt to analyze the performance of 97 District Headquarter Hospitals (DHQ) of Pakistan by calculating economic efficiency (EE) and cost productivity, over the period 2006-15. The research period is divided into two parts i.e. 2006-2010 (pre-decentralization of the age) and 2011-2015 (post-decentralization of the age) to evaluate the change in hospital performance after the 18th constitutional amendment 2010. Data Envelopment Analysis (DEA) and Cost Malmquist Index (CMI) have been used to estimate the economic efficiency and cost productivity of the DHQ hospitals respectively. Bootstrap Truncated Regression has been used to analyze the effects of internal and external factors on economic efficiency and cost productivity of the hospitals. In Punjab, 32% of DHQ hospitals in 2006-2010 and 40% in 2011-2015 have more than threshold level of economic efficiency scores that is 0.70.It is also observed that almost 60% of Punjab hospitals operate under increasing return to scale. Similarly, 27% of DHQ hospitals are recorded to be efficient at Sindh in 2006-10, while 45% in 2011-15. More than 70% of hospitals in Sindh are working under increasing return to scale. The Spearmen rank test reveals that misallocation of resources is the main cause of inefficiency in the hospitals of Punjab and Sindh. The economic efficiency scores of 23% and 32% DHQ hospitals of Khyber-Pukhtunkhwa are higher than 0.70 in 2006-10 and 2011-15 respectively. All the hospitals in Khyber-Pukhtunkhwa are working under increasing return to scale. In the large province of Pakistan, Balochistan, 25% of hospitals are efficient in 2006-10 and 32% in 2011-15. More than 95% of these hospitals are working under increasing return to scale. The result of Spearmen rank test indicates that the wastage of resources is the main reason for inefficiency in the hospitals of Khyber-Pukhtunkhwa and Balochistan. There is not a desired change in the economic efficiency of the hospitals after 18th constitutional amendment 2010. As far as growth in the productivity of the DHQ hospitals in Pakistan is concerned, 15% growth is found during the study period 2006-15. However, the growth rate, 16%, is higher in 2006-10 than, 12%, in 2011-15. At the provincial level, the DHQ hospitals of Baluchistan are relatively more productive followed by Sindh, Khyber-Pukhtunkhawn, and Punjab during 2006-15. The Bootstrap Truncated regression results reveal that all the coefficients of internal and external factors have a significant effect on the x inefficiencies and cost productivity of the DHQ hospitals of Pakistan. The capitalization and doctor to patient ratio have a larger coefficient and a négative impact on economic inefficiency.Size of hospitals and doctor to patient ratios have a comparatively higher and significant effect on cost productivity according to our results. From the perspective of public policy, the Government should try to improve the quality of the management of these hospitals through a dedicated training programme. Also focus should be on reforming medical education to enhance the quality as well as quantity of the medical workforce so that the cost of these hospitals can be reduced. The impact of fiscal decentralization 2010 is found inconclusive. The central government should also revisit the 18th constitutional amendment. In the distribution of health care resources, the government should give priority to underdeveloped areas. To increase the highly specialized medical staff, the government should focus on specialized medical education. Special perks and privileges should be given to those doctors who are posted in far-flung and rural areas to improve medical service provision and quality of healthcare in such remote areas of the country.

افسوس ہے دسمبر کے آخری ہفتہ میں جناب رام بابو سکسینہ کاحرکت قلب بند ہوجانے سے بحالت سفر اچانک انتقال ہوگیا۔ موصوف کو اردو زبان وادب کے ساتھ عشق تھااوروہ اس کا بڑا وسیع اورگہرا مطالعہ رکھتے تھے۔یہی وجہ ہے کہ ایک ذمہ دار سرکاری عہدہ کی چند درچند مصروفیتوں کے باوجود انھوں نے ادبِ اردو پرمتعدد ضخیم کتابیں اپنی یادگار چھوڑی ہیں۔ ان کی کتاب ’تاریخ ادبِ اردو‘ کو بڑی مقبولیت حاصل ہوئی، اس موضوع پر یہ کتاب اس پایہ کی ہے کہ ادبِ اردو کاکوئی طالب علم اس سے بے نیاز نہیں ہوسکتا۔اس کے علاوہ’’اردو کے انگریز شعرا‘‘ نامی کتاب انھوں نے بڑی تلاش وجستجواورمحنت سے مرتب کی تھی۔ایک کتاب حال میں ہی’’مرقع شعراء‘‘کے نام سے شائع ہوئی ہے۔سنا ہے کہ دو کتابوں کے مسودے ابھی اورباقی ہیں جو غیر مطبوعہ ہیں،موصوف اب دنیا میں نہیں لیکن اُن کے یہ شاندار ادبی کارنامے اُن کانام زندہ رکھیں گے۔

Analyzing the Impact of Lady Health Worker Program on Utilization of Maternal Health Services in Community With a Lady Health Worker in Sindh Province

The study aims to analyze impact of Lady Health Worker program on use of maternal health services in community with a lady health worker. The main focus was the rural districts of Sindh for this case study. The maternal health service is determined by at least four Antenatal care (ANC) visits provided to mother and receipt of at least two Tetanus Toxoid (TT) injections to mother during pregnancy. These have direct impact on maternal mortality rate and infant mortality rate. Secondary data pertaining to all districts of Sind for three years (2017-2019) was collected from the Sindh Lady Health Worker program office Hyderabad for analysis purpose. Further secondary data of Pakistan Demographic and Health Survey (2017-18) was also used to see the impact of intervention of LHW program implemented by Ministry of Health Pakistan in 1994 and then devolved to provinces after the implementation of 18th Constitutional Amendment in 2010. The study found that the services provided by the lady health workers in communities in these districts has positively influenced the uptake of maternal health services as determined by the variables like antenatal visits by pregnant mothers to nearest health facilities and receipt of Tetanus Toxoid injection during pregnancy.

Us-Iran Relations After the Islamic Revolution: Impacts on Pakistan

The strong economy and stabilized politics of US and geostrategic location and independent foreign policy of Iran have made them dominate powers in world politics. Unfortunately, their regional contradictory policies have been affectingPakistan badly since Pakistan has been linked with both of them one way or the other. Most importantly, the post-Islamic Revolution period brought serious ambiguities in Pak-Iran-US triangle since Pakistan became a strategic partner of US in Afghanistan against Soviet intervention whereas Iran declared US a Great Devil. Meanwhile, the incident of siege of Makkah and Pakistan’s inclination towards Gulf Cooperation Council (GCC) further caused disturbance in Pak-Iran relations since Iran consider the GCC a council against its ideology. The thesis highlights the involvement of US in the affair of South Asia and Iran and its direct effects on Pakistan after the Islamic Revolution of Iran. It is believed that terrorism, extremism and nuclearization pace in South Asia have created enough excuse for US to butt in the internal politics of South Asia where Pakistan has been witnessed in siding the policies of US whose impacts were directly felt on Pak-Iran relations. The thesis also highlights the questions why has Pakistan been unable to maintain its neutrality over either the Iran-US political conflict or Iran-Saudi ideological war. It is believed that the economic compulsion and destabilized politics of Pakistan led Pakistan to be indecisive over its foreign policies via-a-vis Iran. However, it ought to be noted that the geo-strategic convergence, identical ideology and regional common issues of Pakistan and Iran are certain areas of convergence which do not only bridge the political gap between Pakistan and Iran, they also greatly benefit Pakistan economically and strengthen its national security. Moreover, via Iran, Pakistan can suffice its energy requirement; suppress the extremism and insurgency in Balochistan since these are the common issues of both countries. More importantly, the successful materialization of Gwadar Port and full-fledged operation of China-Pakistan Economic Corridor (CPEC) are mainly depended upon the regional policies of Iran. Therefore, keeping the whole VI above factors, while formulating its regional and international policies, Pakistan has to take a great care of Iran, without harming the national interests of the US, Iran and Saudi Arabia. But, the historical economic dependency towards US and political instability of Pakistan made it extremely tough for Pakistan to create a balanced relationship between US, Iran and Saudi Arabia. So, thesis would give a way forward to Pakistan to escape from the direct conflict of US and Iran and maintain a balanced relationship between Iran and US. The thesis is qualitative in nature where the author has investigated and analyzed certain facts, ambiguities and major issues of US-Iran-Saudi triangle and their implications on the region generally and on Pakistan specifically. The involvement of Pakistan in the proxy war of US in 1980s, becoming the frontline state in war against terror, inclination towards Saudi Arabia after Islamic Revolution, entertaining US regional interests, no doubt, earned dollars for Pakistan, but, they badly enhanced the economic dependency of Pakistan, promoted sectarian violence in Pakistan, doubled terrorist activities in the region and more importantly, created ways for regional and international powers to intervene in the internal politics of Pakistan.
