Background: The shoulder is a complex, multi-joint structure in the human body. It plays a significant role in activities of self-care, work and leisure. Dysfunction and pain in this region can greatly impact a person’s daily life. Within the broad complaint of shoulder pain, rotator cuff disease (RCD) is the most common condition. RCD can cause extensive disruption of physical activity, but also affects mental health and social participation. RCD consists of tendinopathy of one or more of the muscles of the rotator cuff, partial-thickness tears (PTT), or full-thickness tears (FTT) of the rotator cuff tendons. Objective: To determine the effectiveness of low level laser therapy (LLLT) and routine physical therapy (RPT) on pain intensity, shoulder range of motion (ROM), muscle strength, quality of life, functional disability, supraspinatus tendon thickness and tear thickness in patients with partial tear of supraspinatus tendon. Materials and Methods: 72 patients with confirmed diagnosis of partial tear of supraspinatus tendon through musculoskeletal ultrasound (MSKUS) were randomly allocated into two groups by using a computer generated random sequence table with 36 patients in the experimental group (LLLT+RPT) and 36 patients in the control group (RPT alone). Baseline assessment for pain intensity, shoulder ROM, muscle strength, quality of life, functional disability, supraspinatus tendon thickness and tendon tear thickness were recorded using Visual Analogue Scale (VAS), Universal Goniometer (UG), Oxford Scale for Manual Muscle Testing (MMT), EQ-5D, Disability of Arm, Shoulder and Hand (DASH), and MSKUS respectively, before treatment. All the patients received 18 treatment sessions over a period of 6 weeks (3 sessions per week) and 12 further sessions from 6weeks to 12 weeks follow up (2 sessions per week). At the end of six weeks and 12 weeks, all the outcome measures were assessed again. Results: Repeated Measure Analysis of variance (ANOVA) revealed within groups, significant differences in the mean improvement in all outcome measures in experimental group both at 6 and 12 weeks follow-up. Significant differences were also found in control group for the mean improvement in all outcome measures except non-significant in tendon thickness, tear thickness and supraspinatus strength at 6 weeks whereas significant differences were found in control group for the mean improvement in all outcome measures except non-significant in tendon thickness, tear thickness, supraspinatus strength and deltoid strength at 12 weeks follow-up. An independent t-test revealed between two groups, at 6 weeks, significant differences were found for the mean improvement in tendon thickness, VAS, ROM, QOL and DASH except nonsignificant for tear thickness, supraspinatus strength and deltoid strength, whereas, at 12 weeks, significant differences were found in mean improvement for all outcome measures. However, there was more improvement in the experimental group compared to the control group. Conclusion: The findings of the present study revealed that RPT with and without LLLT was equally effective for all outcome measures for both groups except tendon thickness, tear thickness, supraspinatus strength and deltoid strength for control group in patients with partial tear of supraspinatus tendon.

افسوس ہے دسمبر کے آخری ہفتہ میں جناب رام بابو سکسینہ کاحرکت قلب بند ہوجانے سے بحالت سفر اچانک انتقال ہوگیا۔ موصوف کو اردو زبان وادب کے ساتھ عشق تھااوروہ اس کا بڑا وسیع اورگہرا مطالعہ رکھتے تھے۔یہی وجہ ہے کہ ایک ذمہ دار سرکاری عہدہ کی چند درچند مصروفیتوں کے باوجود انھوں نے ادبِ اردو پرمتعدد ضخیم کتابیں اپنی یادگار چھوڑی ہیں۔ ان کی کتاب ’تاریخ ادبِ اردو‘ کو بڑی مقبولیت حاصل ہوئی، اس موضوع پر یہ کتاب اس پایہ کی ہے کہ ادبِ اردو کاکوئی طالب علم اس سے بے نیاز نہیں ہوسکتا۔اس کے علاوہ’’اردو کے انگریز شعرا‘‘ نامی کتاب انھوں نے بڑی تلاش وجستجواورمحنت سے مرتب کی تھی۔ایک کتاب حال میں ہی’’مرقع شعراء‘‘کے نام سے شائع ہوئی ہے۔سنا ہے کہ دو کتابوں کے مسودے ابھی اورباقی ہیں جو غیر مطبوعہ ہیں،موصوف اب دنیا میں نہیں لیکن اُن کے یہ شاندار ادبی کارنامے اُن کانام زندہ رکھیں گے۔

The Risk of Developing Central Line-Associated Bloodstream Infections in Integrated Health Care System of Pakistan-A Meta-Analysis

Studies regarding the prevalence of CLABSIs in Pakistan are limited. However, it is known that healthcare-associated infections are a concern in many low- and middle-income countries, including Pakistan. The present study is aimed to identify the relative risk of developing CLABSIs in the hospital care setups of Pakistan. The risk of biases in included studies was assessed using Cochrane tool parameters. Analysis of results revealed a relative risk of getting CLABSIs is 1.78 (p<0.001) among patients admitted in the hospitals for greater than 72 hours. This shows that the chances of getting infected at the central line site were more than 50% among patients admitted to ICUs in Pakistan. It has been concluded that the relative risk of CLABSIs in the integrated healthcare system of Pakistan is high.   DOI: https: //doi. Org/10.59564/amrj/01.01/003  

Implementation of an Integrated Curriculum

This study is based on the investigation of the process of implementation of an integrated curriculum. The fieldwork for the study was done in one of the private school Karachi, Pakistan. A total of seven participants and three classrooms for observations were included in this study. Interviews, observations and document analysis were the major tools for collecting data. As a result of the qualitative analysis of the data, the study has come up with some major findings. Some of the findings relate well with the literature and some tend to go in a different direction. In the present context of the school, the concept of integrated curriculum is not clear in the minds of the teachers. Teachers say they use activities and do practical work to teach our children but observation has shown it is difficult for them to do practical work. They are using teacher-directed method in the classroom. It is difficult for a single teacher to manage practical work. The activities are mostly paper-pencil work. Teaching through integrated curriculum is a difficult task in the Pakistani context, where teachers are transmitting knowledge to the children. Teachers need to be given the clear understanding of the task and its objectives; otherwise the implementation will not be successful. For successful implementation the whole staff in the school, especially the teachers, needs to be involved. Without their involvement it is difficult to de design/plan a curriculum, where children are given more opportunities to participate in the class rather than teachers doing everything. For an integrated curriculum teachers see at the needs and interest of the child and select the themes for their study. In the present practice, the curriculum manual has been given to the teachers to follow exactly as it is with activities as well, no teacher involvement has occurred. Teaching through an integrated curriculum requires teachers to play different roles such as a facilitator, a planner and a guide. But the present situation in the school shows the teacher to be a dictator. Classes are teacher-centered instead of being child-centered. Teacher's role needs to guide and help children whenever they are in need rather than simply giving instructions. There are some challenges as well that the school is facing. The major challenge is assessment in an integrated curriculum. The school is struggling as to how to do t assessment. They were doing daily assessments two years back when they
