Milnacipran HCl is a potential candidate to treat fibromyalgia associated with or without depression like comorbid conditions. The requirements of multiple dosing, absorption in first part of small intestine and fluctuating steady state plasma concentrations of drug have made it a suitable candidate for gastro-retentive sustained release drug delivery system. The utility of chitosan as mucoadhesive and polycaprolactone as hydrophobic polymer was aimed for creation of this matrix delivery system. Modified hot melt granulation with addition of second polymeric solution was opted for preparing optimized blends of acetaminophen. Modified wet granulation technique was utilized to prepare blends of milnacipran with different ratios of polymers. The optimized solutions for formulations containing blends of three polymers and drug with two solvents, i.e., dichloromethane and chloroform was optimized using design of experiment. A simple and sensitive colorimetric method for estimation of milnacipran was developed with ninhydrin to properly estimate drug from dissolution samples at 570 nm wavelength. The optimized blends were characterized for mucoadhesive time and strength using tensiometer, drug release analysis opting HCl buffer pH 1.2 for 12 h, kinetics of drug release and drug excipient compatibility analysis by utilizing FTIR and DSC. The optimized formulations M21-M24 were compared against reference formulation specifications for similarity index (f1 and f2). The optimized formulations were also scaled down containing 30 mg of drug according to weight of animals and were characterized for release parameters, i.e., the time to release certain percentage of drug (T10%-T90%). Different pharmacokinetic parameters were evaluated for M44 formulation using both non-compartment and two compartment model approaches. The best correlation (R2) values of 1 were observed with non-compartment model so parameters calculated were used to establish in-vitro in-vivo II correlation after estimating fractions of milnacipran absorbed/unabsorbed and time to absorb specific concentration of milnacipran using Wagner-Nelson method. The developed analytical method proved sensitive and precise with LOD and LOQ values of 0.55 and 1.667 μg/ml, respectively. Mucoadhesive strength was found as 201.06±16.8 dynes for different formulations with more than 12 h of adhesion time. Drug release studies showed release independent of concentration and was best described by Korsmeyer-Peppas model representing swelling controlled system. The n values of 0.5-0.7 showed dependency of release on both dissolution and diffusion mechanism. There were only physical interactions observed at various positions of IR spectrum with functional groups of polymers and drug were found intact. Thermal studies revealed that milnacipran has sharp melting point of 179.85℃ which was lowered to round about 154.30℃ due to covalent and ionic interactions required to form true blend with polymers. M24 formulation proved best on similarity parameters, i.e., f1=6.09, and f2=72.46, representing true similarity. Cmax, Tmax, and AUC0-∞ calculated for M54 were markedly found different from oral solution (P˂0.05) proving sustained delivery of milnacipran from the formulation blends. Bioavailability of M54 formulation was three fold improved as compared to solution. Level A correlation was established by comparing the above parameters and found R2 were 0.992, 0.9831 and 0.9998. The above results proved that milnacipran might be formulated as mucoadhesive DDS using three polymers opting design of experiment approach and evidenced best on IVIVC. III

مرحوم خلیق ٹونکی فن خطاطی میں اعلیٰ شہرت کے مالک تھے اس کے علاوہ مرحوم میں بڑی خوبیاں تھیں، صوم وصلٰوۃ کے پابند تھے، منکسر المزاج تھے، اخلاق کریمانہ کے مالک تھے۔ اور مفکرمِلّت حضرت مفتی عتیق الرحمن عثمانی ؒ کے شیدائی اور معتقد تھے۔ حضرت مفتی صاحبؒ نے ان کے فن کونکھارنے میں بڑا تعاو ن کیا۔ ادارہ ندوۃ المصنفین اوراس کے رسالے ’’برہان‘‘ ہی سے انہوں نے فنِ کتابت کی ابتدا ء کی۔ قبلہ مفتی صاحبؒ نے ادارہ ندوۃ المصنفین اوراس کے رسالے ’’برہان‘‘ کے لیے کتابت وطباعت میں ہمیشہ ہی اعلیٰ معیار کو برقرار رکھنے کی بھرپور کوشش کی۔اسی لیے انہوں نے ہندوستان کے اعلیٰ فنکاروں کوکھوج کھوج کرچُن چُن کرادارہ میں جمع کیا۔جناب آل حسنؒ، جناب محمد یوسفؒ، جناب عبدالقیوم ؒ اور خلیق ٹونکی کوبحیثیت کاتب کے ادارہ ندوۃ المصنفین میں رکھا۔ اﷲ تعالیٰ کے فضل سے خلیق تونکی کتابت کی دنیا میں شہرت کی بلندی پر پہنچے، سرکاری ایوارڈ بھی انھیں حاصل ہوئے، ملک اور بیرونِ ملک میں ان کے ہزاروں شاگرد موجود ہیں۔ادارہ ندوۃ المصنفین کی کئی کتابیں ان ہی کی کتابت کی ہوئی ہیں۔ ادارہ کی مشہور کتابیں’ مصباح اللغات‘ اور’ قصص القرآن‘ جناب خلیق ٹونکی ہی کی کتابت کی ہوئی ہیں۔
انھیں ہمیشہ ہی اس بات کا اعتراف رہاکہ ان کی کامیابی وشہرت میں بفضلِ خدا حضرت مفتی عتیق الرحمن عثمانیؒ اوران کے ادارہ ندوۃ المصنفین کابڑا ہاتھ رہاہے۔اسی لیے وہ ہمیشہ ہی ادارہ ندوۃ المصنفین اوراس کے ڈائرکٹر راقم الحروف عمیدالرحمن عثمانی سے زندگی کے آخری لمحے تک والہانہ لگاؤ وعقیدت رکھتے تھے۔

Health Care and Cleanliness in Tertiary Care Hospitals in Peshawar, Khyber Pakhtunkhwa

The present study was conducted to probe the cleanliness in the tertiary healthcare hospitals in Peshawar-Pakistan. The data was collected through a structured questionnaire from 600 sampled respondents admitted in three tertiary healthcare hospitals in Peshawar i.e. Khyber Teaching hospital, Hayatabad Medical Complex and Lady Reading Hospital through proportional allocation method. The patients were very critical about the healthcare and cleanliness in the above three major hospitals of Khyber Pakhtunkhwa. The Bi-variate analysis was made and the results reveal that hospital has clean and hygienic environment (P=0.006), Medical OPD, s were clean and tidy (P=0.000), wards were clean and well maintained (P=0.000), Toilets were clean and well maintained (P=0.000). The study recommended that hospital administration should keep focus on cleanliness in washroom, clean water and clean food to the patients.

Problems of Relapse into Illiteracy of Adult Female Neo-Literates in Punjab and Development of a Strategy for its Prevention

This study attempted to assess the retention in basic skills of literacy of neo- literates. It also explored the problems of relapse into illiteracy of adult female neo- literates; the possible causes behind the problem of relapse; and, development of a strategy for prevention of relapse into illiteracy. The study looked at neo-literates about three years after they had completed their literacy courses. Two main organisations launching programmes of adult literacy for females in Punjab were considered. The sample of the study involved 1099 respondents that were chosen through multi steps sampling techniques from 9 districts of the Punjab. The research questions were formulated to give a focus for the study. A mixed method research design (concurrent nested design) was applied in order to achieve the objectives of the study. A test was conducted to determine the retention level of neo-literates in reading, writing and numeracy skills. The researcher developed a questionnaire for teachers and supervisors of adult literacy programmes, an interview schedule for neo-literates, an interview schedule for high officials and a list of themes for Focus Group Discussions (FGDs). The views of teachers and supervisors were compared using a chi-square test of independence. The data of interview schedule of neo-literates were analyzed by categorizing, coding and were presented in tabular form with frequencies and percentages. The qualitative data of interview schedule of high official and FGDs were analyzed on the basis of individual statements. It was found that only 17% of neo-literate of National Commission for Human Development (NCHD) could perform better in reading skills, 13% in writing skills and 5% in numeracy skills of adult literacy program after two year of their study. In viiiDepartment of Literacy and Non-Formal Education Punjab (DLNEP) only 6.5% of neo-literate could perform better in reading, 7.5% in writing, and 5.8% in numeracy skills. The adult female neo-literates faced major problems in basic reading and even greater problems in writing and numeracy. Thus, problems included: reading and writing alphabets, combined words having three or more alphabets, sentences, signboards to locate places, applications and letters. Problems related to relapse in numeracy skills were: counting up to 100, solving sums of addition, subtraction, multiplication and division, making home budget and teaching to students of classes 1 and 2. Thus, the teachers were inadequately trained and failed to observe the basic standards of courtesy and respect for the learners. The courses ran at inappropriate times, were too short and used inadequate accommodation. While not excessively difficult, the materials used were not related to the real needs of the learners who wished, in the main, for literacy skills in relation to income generating activities. There was a need for increased opportunities to practise the skills taught and follow up courses were required. It was suggested that literacy programmes may be designed as per demand and need of the learners, income generating component may be included and taught in an integrated way; post literacy programmes may be offered to consolidate and even strengthen the basic skills of literacy.
