The rapidly growing drug resistance among bacterial strains has become a challenge for scientific community for treating life-threatening infections. In the past few years, the extensive and indiscriminate use of broad spectrum drugs has led to the emergence and spread of multiple drug resistance among bacterial flora. β-lactam drugs are considered as the most widely prescribed antibiotics for treating infections caused by gram-negative bacterial strains of Enterobacteriaceae family.The emerging resistance in the strains of Enterobacteriaceae family has been observed due to the production of various resistance enzymes like beta lactamases, extended spectrum beta lactamases and carbapenemases. In this study, our primary focus was to study drug resistance amongst strains of the genus Klebsiella. Klebsiella species have gained significant clinical importance due to high rates of mortality and morbidity among the nosocomial and community acquired infections caused by them. Resistance enzymes like beta lactamases, extended spectrum beta lactamases and carbapenemases producing strains of Klebsiella species are capable of hydrolyzing Beta Lactam, Extended Spectrum Cephalosporin and Carbapenem drugs respectively. Studies have revealed that these enzymes produced by Klebsiella species are found to be plasmid mediated and can confer resistance from one bacterial strain to another via horizontal gene transfer. They can also mediate resistance against antibiotics other than beta lactams including aminoglycosides, quinolones, fluoroquinolones, etc. In our study, we performed a comprehensive resistance profiling of 125 Klebsiella strains and detected the prevalence of resistance enzyme producers by phenotypic and genotypic methods. Drug susceptibility testing was performed by Kirby Bauer Disc Diffusion method and minimum inhibitory concentrations were also determined by Macrobroth Dilution method. Phenotypic detection of Extended Spectrum Beta Lactamase (ESBL) was performed by the techniques of Double Disc Synergy and Combination Disc Test, whereas production of Carbapenemase enzyme was detected by Modified Hodge Test.Genotypic characterization of drug resistant Klebsiella strains was performed by identifying the prevalence of resistance genes including blaTEM, blaSHV, blaCTX-M blaNDM, blaKPC, blaOXA-23, blaOXA-51, and AmpC. Polymerase chain reaction (PCR) was performed for the detection of resistance genes and DNA sequencing was performed for selected gene to verify the variant of the gene. In our study, we identified multiple drug resistant strains of Klebsiella, with high resistance potential against Amoxicillin (100%), Tetracycline (86%), Nalidixic Acid (86%), Cephradine (82%), Gentamicin (82%) and Co-amoxyclav (80%) respectively. Antimicrobial drugs that showed good inhibitory results against Klebsiella strains were found as Amikacin (96%), Meropenem (94%) and Piperacillin-tazobactam (91%). Minimum inhibitory concentrations (MICs) were determined to validate the susceptibility results obtained through disc diffusion method. Resistance percentages with Macrobroth dilution method were detected as 81.6%, 81.6%, 63.2%, 36%, 28.8% and 25.6% against gentamicin, cephradine, co-amoxyclav, cefuroxime, ciprofloxacin, cefotaxime and cefoperazone respectively. Carbapenems were found as the most effective drugs against Klebsiella isolates. The drug resistant Klebsiella strains were further analyzed phenotypically to detect the production of extended spectrum beta lactamase and Carbapenemase enzymes. It was found that 64.8% and 5% of Klebsiella isolates were extended spectrum beta lactamase and carbapenemase producers, respectively. Furthermore, we performed molecular detection of drug resistance genes to confirm phenotypic findings of our study. The polymerase chain reaction method revealed the presence of resistance genes predominantly blaSHV, followed by blaTEM, blaKPC, AmpC, blaOXA-51, blaCTX-M, blaNDM-1 and blaOXA-23 in 48%, 43.2%, 38%, 23%, 3.2%, 2.4%, 2.4% and 1.6% in Klebsiella isolates, respectively. We randomly selected a drug resistant ESBL producing Klebsiella strain to identify the variant of the gene through DNA sequencing. Complete sequence of the blaTEM positive PCR product revealed 94% similarity with TEM-198 of Klebsiella pneumoniae KUN5033.In this study, we determined high prevalence of drug resistance among Klebsiella strains due to the production of plasmid mediated enzymes predominantly extended spectrum beta lactamase. It was found that these resistant strains cannot be easily detected through phenotypic screening methods. Therefore, identification and confirmation of these strains through molecular methods are preferred. This study provides the phenotypic and genotypic statistical data of drug resistant Klebsiella strains circulating among the population of Karachi-Pakistan, which will facilitate the clinicians to design a correct empirical therapy and to implement infection control policy.

                تغیر و تبدل انسانی زندگی کا خاصہ ہے اور تخلیقِ آدم سے ہی شاید یہ سلسلہ شروع ہو گیا تھا۔اگر مختلف تہذیبوں کی تاریخ کا مطالعہ کیاجائے تو یہ ثابت ہوتا ہے کہ تہذیبوں کی تبدیلی کے ساتھ ساتھ زبان وادب بھی متاثر ہوتا رہا اور اس میں بتدریج تبدیلیاں ہوتی رہیں۔یہ ایک مسلمہ حقیقت ہے کہ ادب معاشرے کو متاثر کرتا ہے اور معاشرہ ادب پر اثر انداز ہوتا ہے۔ اُردو زبان بھی ابتدائی دور میں فارسی ادب سے زیادہ متاثر ہوئی۔ اُردو کی ابتدائی شاعری کے مطالعہ سے پتا چلتا ہے کہ ہمارے شعراء نے بھی وہی گل وبلبل کے قصے اور محبوب کی بے وفائی کو شاعری کا موضوع بنایا۔ اگرچہ مرزا مظہر علی خاں کو بعض ناقدین اُردو شاعری کی کثافت کو کم کرنے کا معمارِ اوّل سمجھتے ہیں اور میرتقی میرؔ نے بھی یہ دعویٰ کیا تھا۔

                اِس بات پر تمام ناقدین متفق ہیں کہ اُردو شاعری میں جدت کی بنیاد مولانا الطاف حسین حالیؔ نے رکھی۔ اگرچہ اِس سے پہلے نظیر اکبرآبادیؔ بھی شاعری کو اُمرا کی محفلوں سے نکال کر غرباء کے مساکن اور گلی محلوں تک لے آئے تھے۔ مگر حالی وہ شخصیت ہیں جنہوںنے اس بنیاد پر بہت بڑی عمارت کھڑی کردی۔ مولانا الطاف حسین حالیؔ اس حوالے سے ’’مجموعہ نظم حالی‘‘ میں جدید اردو شاعری میں فطرت نگاری، مقصدیت اور سادگی کو شاعری کا لازمہ قرار دیتے ہیں۔(۱)

Comparative Study of Organizational Commitment, Morale and Job Satisfaction of Subject Specialists and Lecturers Teaching at Intermediate Level

This study was aimed at comparing the organizational commitment, morale and job satisfaction of Subject Specialists and Lecturers teaching at intermediate level. The major objectives of the study were to compare the level of organizational commitment, morale and job satisfaction of Subject Specialists and Lecturers and also to determine the relationship among organizational commitment, morale and job satisfaction of Subject Specialist and Lecturers. The differences in the level of organizational commitment, morale and job satisfaction of Subject Specialists and Lecturers were calculated and relationship among their organizational commitment, morale and job satisfaction was found. The study was delimited to Subject Specialists of Government Higher Secondary Schools and Lecturers of Government Colleges working on regular basis, in the jurisdiction of the Khyber Pakhtunkhwa province, Pakistan. It was an inferential study and the survey method was used for data collection. The population for the study was all the Subject Specialists of 292 Government Higher Secondary Schools and the Lecturers of 177 Government Colleges of Khyber Pakhtunkhwa province. Using the single-stage duster sampling technique a sample of 610 Subject Specialists from 95 Government Higher Secondary Schools and 526 Lecturers from 45 Government Colleges was selected. Three instruments i.e. Organizational Commitment Scale (OCS) adapted from Organizational Commitment Questionnaire by Mowday, Steers, and Porter (1979), Morale Scale (MS) adapted from Purdue Teacher Opinionaire used by Houchard (2005) and Job Satisfaction Scale (JSS) adapted from Scale of Job Satisfaction by Bas and Ardic (2002) were used for the collection of data in this study. All the three instruments were validated by seven experts and administered to 145 respondents (74 Subject Specialists and 71 Lecturers) for pilot testing. Data were collected through vitipersonal visits. The number of respondents in this study was 951 (542 Subject Specialists and 409 Lecturers). Data were analyzed by applying two-sample t-test and the Pearson’s correlation test. The major findings of the study were that Lecturers showed a higher level of organizational commitment, morale and job satisfaction as compared to Subject Specialists. There was no difference in level of organizational commitment, morale and job satisfaction between the urban and rural Subject Specialists; the rural Lecturers have higher level of morale and job satisfaction than urban Lecturers while no difference was found in their level of organizational commitment, Both die male and female Subject Specialists as well as Lecturers showed no difference in their level of job satisfaction. Bui male Subject Specialists and Lecturers both showed higher level of organizational commitment than their female counterpart. On the other hand in case of morale, male and female Subject Specialists showed no difference while male Lecturers have higher level of morale than females. The Subject Specialists having more than 7 years experience showed higher level of organizational commitment and job satisfaction than the less experienced Subject Specialists while no difference was found in their level of morale. On the other hand Lecturers showed no difference in all the three variables i.e. organizational commitment, morale and job satisfaction with respect to experience and qualification. The Subject Specialists having only Master degree showed higher level of morale and job satisfaction as compared to those having higher qualification of M. Phil, and Ph, D., while no difference was found in their level of organizational commitment with respect to qualification. A strong positive relationship was found among organizational commitment, morale and job satisfaction for both the cadres of Subject Specialists and Lecturers. It was recommended that enough physical facilities, conducive working ixenvironment and higher qualification allowance might be given to Subject Specialists, there was need to change the physical facilities and working environment in the Higher Secondary Schools and higher qualification allowance may be awarded to Subject Specialists.
