Breast cancer has been identified as the most common malignancy among Pakistani women, accounting for 34.6% of all female cancers. Around 80-90% cases are sporadic in nature. Multiple factors, including genetic as well as epigenetic factors are responsible for the initiation of breast cancer. In the present study, we carried out genetic characterization of breast cancer in our local population. Diagnosed breast cancer patients having a mean age of 53yrs were included in this study. These patients were analyzed for mutations in BRCA1 and two metastasis suppressor genes (KAI1 and Kiss1). Germline mutations of three genes BRCA1, Kiss1 and KAI1 were investigated in already diagnosed sporadic breast cancer females. BRCA1, a tumour suppressor gene is found responsible for majority of breast cancer cases of familial history. However its involvement in sporadic breast cancer cases (especially of Pakistani population) is still an area that requires further research. BRCA1 proteins binding domain encoding regions were screened in this regard. Five silent mutations along with one novel splice site mutation were observed on BRCA1. However, most of the BRCA1 portions analyzed remained conserved at genomic levels. This study thus indirectly highlights the involvement of other signaling cascade molecules as well as transcriptional and translational regulatory mechanism responsible for breast cancer. Metastasis is among one of most lethal attribute of cancer. In this study, two metastasis suppressor genes (Kiss1 and KAI1) were screened for any germline mutations/polymorphism in these patients. So far no previous findings with respect to these genes have been reported. In our study no exon or introns deletions had been observed on these genes. Altered expression (down- regulation) of these genes has extensively been reported in the literature in relation to different types of cancers. However, expressional correlation of KAI1 with disease progression is established for the first time by this study. Significant decrease in KAI1 expression has been observed in localized and invasive cancer cells when compared to normal cells. Significant correlation of KAI1 with TNM staging (Tumour, node and metastasis) has also been established (p=0.045).Patients showing higher expression of KAI1 showed better survival rate (after median follow up of 120 months) when compared to those showing less or almost negative expression (p=0.0136). Thus potential involvement of KAI1 molecule as a marker of prognostic significance is established. Interaction of KAI1, tetraspanin family member, with FAK, Shh, EGFR and integrins was done as preliminary screening to find its potential involvement in the metastatic cascade. A chain of in vitro assays were conducted to explore KAI1 cellular response. Two transgenes based effective knock outs (MDA-MB- 231 KAI1KO1 , MDA-MB-231 KAI1KO2 ) and one forced expression (MCF-7 KAI1Exp1 ) were successfully generated through the aid of molecular tools. Expression levels of KAI1 in both MDA-MB-231 and MCF-7 were confirmed through RTPCR and western blot. No significant association of KAI1 on cancer cells growth had been observed. This shows that KAI1 does not influence cell proliferation. However, a direct relationship of KAI1 with intracellular attachment to basement membrane has been established in both knock out as well forcedly expressing breast cancer cell lines. Increased expression of KAI1 resulted in strong cells adhesion to basement membrane in MCF-7 KAI1Exp1 when compared to both wild type and controls (MCF-7 Wt , MCF-7 Ct ) with p value of 0.021. Similarly weak adhesion of cancer cells has been observed in MDA-MB-231 KAI1KO1 and MDA-MB-231 KAI1Ko2 when compared with MDAMB-231 Wt and MDA-MB-231 Ct ) with p values of 0.002 and 0.0004 respectively. A significant increase in cancer cell invasion was observed in both ribozymes based KAI1 knock outs (MDA-MB-231 KAI1KO1 and MDA-MB-231 KAI1Ko2 in relation to control (MDA-MB-231 Ct ) with p values of 0.0063 and 0.007 respectively. Furthermore, MCF-7 KAI1Exp1 cells showed reduced cell invasiveness in relation to MCF-7 wt and MCF-7 Ct (p=0.022). Cell motility was also analyzed in both absence and forcedly expressing KAI1 cell lines. MDA-MB-231 KAI1KO1 and MDA-MB-231 KAI1KO2 showed restricted cell migration (p=0.003 vs control) towards the wound. Increased cellular migrations in time lapse based video (up to 90min) recording of MCF-7 KAI1Exp1 was observed (p-0.024 vs control). This study has shown that both genetic and epigenetic factors are mainly responsible for systematic progression. Although the contributory role of gene mutations is low but expressional regulation of these genes is an area which can help in identifying the missing links. Establishing a marker of prognosticsignificance as well as its potential role in metastatic cascade suppression can help in designing most promising gene therapy for breast tumour affected patients.

Factors Related to the Implementation of Evidenced-based Practice (EBP) of Nursing Staffing Levels in Hospitals: A Literature Review

The implementation of Evidenced-based practice (EBP) was one of the factors in increasing the quality of health services as an ideal problem-solving approach. The ability of nurses to appliance EBP was influenced by many factors, not only on individual nurses consisting by the support of unit leader and colleague. The purpose of this study was to determine the factors associated with the implementation of EBP at the care room level establishing on the support of coworkers and nursing managers. This writing used a literature review study, from several databases namely PubMed, ProQuest, Google Scholar, and ScienceDirect with a limitation of the last 5 years. 17 journals met the inclusion criteria and the results of a literature review study show: eight journals conclude that the support of colleagues and authority in changing practices affects the implementation of EBP, 13 journals concluded the role of managerial nurses affects the implementation of EBP consisting of leadership support and mentorship including the lack of knowledge and skills of nursing leaders regarding EBP. Data from the collected and identified literature study clearly showed that EBP knowledge and skills of nursing leaders related to EBP are important factors in increasing the implementation of EBP but co-worker factors can be a supporter and also an obstacle to the implementation of EBP if it is not well managed.

Management of Pink Bollworm Pectinophora Gossypiella Saunders Lepidoptera: Gelechidae Through Ipm Strategies

Pink bollworm, Pectinophora gossypiella (Saunders) is one of the serious insect pests of cotton crop attacking fruiting bodies which may result in loss of yield and damage to cotton fibre quality. In the present study comparative resistance of Bt. and non-Bt. cotton varieties was evaluated for management of P. gossypiella. Different Bt. cotton varieties, IR-901, FH-142, MNH-886, NIBGE IR 3701, CIM-602 and Tarzan-1 containing (Cry1Ac) and non-Bt. varieties, CRIS-134, CRIS-342, CRIS-129, CRIS-613, FH-1000 and FH-901were used in the study during two consecutive years (2015 and 2016) at district Sanghar, Sindh, Pakistan. The results indicated that the maximum attack of P. gossypiella larvae was recorded on IR-901 and minimum CIM-602 in Bt. cotton varieties. However, in non-Bt. cotton, maximum attack was observed on FH-901 and minimum on CRIS-129. Biological parameters of P. gossypiella on Bt. and non-Bt. cotton varieties were evaluated under laboratory conditions. The result revealed that the larval period and pupal weight, adult longevity and female reproduction rate of P. gossypiella were significantly lower on Bt. cotton as compared to non-Bt. However, there was no significant difference in pupal period between Bt. and non-Bt. cotton. The larval % parasitization of P. gossypiella was recorded by collection of P. gossypiella larvae from cotton field and ginning factories. Four parasitoids i.e., Tachinid sp, Apanteles sp, Habrobracon sp and Magasillia sp found parasitizing the larvae. The maximum incidence of parasitization was observed in cotton field as compared to cotton ginning factories. The present study also monitored the post-harvest population of P. gossypiella in different cotton ginning factories from three locations of district Sanghar such as Jhol, Sanghar and Shahdadpur. The results revealed that the maximum larval population was recorded from Sanghar and Shahdadpur. Maximum P. gossypiella adult population by using synthetic sex pheromone traps was recorded at Jhol as compared to Sanghar and Shahdadpur cotton ginning factories. The maximum mean larval and adult P. gossypiella population was observed in the months of October and November. Light and synthetic sex pheromone traps were installed in Bt. and non-Bt. cotton field to monitor population of P. gossypiella. The results showed that the highest population of P. gossypiella males was captured from non-Bt. as compared to Bt. cotton through light and synthetic sex pheromone traps. The maximum population was trapped in the month of September and October in both years. The regression analysis indicated that P. gossypiella adults had negative and non-significant relation with temperature and positive and significant relation with relative humidity. Different plant extracts and insecticides were tested in the fields on Bt. and non-Bt. cotton crop for the management of P. gossypiella. The highest reduction percentage of P. gossypiella larvae was recorded on tobacco treated plot followed by neem and datura in both Bt. and non-Bt. cotton crops. The effect of varietal resistance of (Bt. and non-Bt. cotton varieties) was not recorded in 2015 but in 2016 the effect of tobacco and Bt. cotton on larval population was significant. However, the application of different insecticides (triazophos, spintoram and profenophos+cypermethrin) on Bt. and non-Bt. cotton crop indicated that the maximum pest population reduction was recorded in triazophos followed by spintoram and profenophos+cypermethrin treated plots in both Bt. and non-Bt. cotton.
