Platelet flow cytometry is an emerging tool in diagnostic and therapeutic hematology. It is eminently suited to assess the expression of platelet surface receptors both qualitatively as well as quantitatively. It can thus serve as a useful parameter for in vivo expression of platelet activation and thus fore-warn the risk of thromboembolism which is inherent in patients with untreated type 2 diabetes mellitus. This technique can also be used to study and compare the effect of various antiplatelet drugs on the level of platelet activation and to establish any dose-effect relationship of these drugs. Topographical localization of platelet granules and study of platelet-platelet and platelet-leukocyte interaction is also possible by this procedure. All these parameters serve as pointers towards the presence of activated platelets in the circulation with its thromboembolic consequences. This technique can also be put to therapeutics by assessing the effect of varying doses of clopidogrel on suppressing platelet activation. It can thus serve as a reliable and easy technique to determine the dose of various antiplatelet drugs and offer a custom-made protective antithrombotic therapy. This study was undertaken with the primary object of determining platelet reactivity in patients with untreated type 2 diabetes mellitus and to study the effect of clopidogrel administration on the state of in vivo platelet XIactivation in these patients. It was extended to include the effect of clopidogrel on reactive platelet CD markers after in vitro pre-treatment with platelet agonists, more specifically ADP. This agonist was selected because of its easy handling and predicable platelet stimulating effect. It was postulated that platelets in patients with type 2 diabetes mellitus circulate in the blood in hyperactive state and predispose to thrombotic complications so frequently encountered in these patients. This study confirms that platelets in these patients are at a higher state of reactivity and that clopidogrel can effectively suppress the enhanced platelet response. Four CD markers were chosen for this study; of these four markers CD63 and CD62p have a linear relationship with heightened platelet reactivity. The other two markers i.e. CD41 and CD61 are unaffected in these patients and do not seem to be causally related to increased propensity to develop athero-thrombotic complications in patients with type 2 diabetes mellitus. Sequential study of the reactive CD markers i.e. CD63 and CD62p shows that CD63 hyperactivity as indicated by flourescine positivity is a constant and persistent change while that of CD62p is an unstable response which falls off with the passage of time. It is therefore concluded that CD63 platelet markers should be evaluated to assess the state of platelet reactivity in patients with untreated type 2 diabetes mellitus.

دراسة تحقيقية حول حياة الإمام نوح بن أبي مريم الجامع جرحا وتعديلا

Al-Imam Nooh Ibne Abi Maryam Aljam'a is one of the well known worldly personality among the International Scholars of Science of Hadith. Unluckily, his positive personality (Ta'deel) in the Science of chain of Narrators (Ilm-e-Asmaur Rijal wa See'r) had been hidden from the sights of worldly known scholars since centuries. This research contribution is based on manuscript of Al-Istighna fee Asma el Mashooreen Bilkuna by Hafiz Ibne Abdul Bar Al-Qurtubi Al-Maliki (death: 460 hijri) found from Mufti Ilahi Bakhsh Library Kandhala India، despite of the Jarah of other scholars. The manuscript is written by Al-Hafiz Abi Abdullah Muhammad bin Abil Fatah Al-Ba'li Alhanbali (Teacher and Mentor of Al-Hafiz Al-Zahabi) (death: 709 hijri). This contribution tries to explore the personality of the great scholar as well as shower a spot light on the critic of scholars and highlight the reality of this Imam.

Evaluating the Diagnostic Performance of an Initial Transvaginal Sonogram Tvs in Patients With Suspected Ectopic Pregnancy

Purpose of the study: To evaluate the diagnostic performance of an initial transvaginal sonogram (TVS) in patients with suspected ectopic pregnancy (EP). Objectives: 1) To determine the accuracy, sensitivity, specificity, positive and negative predictive values of an initial TVS scan in patients with suspected EP. 2) To determine the diagnostic utility of TVS findings as predictors of EP. Methodology: This was a descriptive, prospective study of 131 consecutive mothers who had TVS for clinical suspicion of EP at the Aga Khan University hospital over a 12 month period between November 2007 and October 2008. The scans were performed in our radiology department by sonographers, residents and consultant radiologists. The mothers were followed-up to determine their clinical or surgical outcome, which was then used as a reference standard. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value for the initial TVS and sonographic predictors of EP were calculated. The study was approved by the University Research and Ethics Committee. Results: The accuracy of an initial TVS in this study was 96% (126 / 131). Sensitivity, specificity, PPV and NPV of initial TVS was 84% (21 / 25), 99% (105 / 106), 95.5% (21 / 22) and 96.3% (105 / 109) respectively. The prevalence of EP in our study was 19.08 % (25 / 131). In this study, 69.4% (91/131) of mothers had intrauterine pregnancies while 10.4% of them were not gravid. Adnexal mass, hemoperitoneum and tubal ring sign had high specificities but low sensitivities. Presence of an extrauterine gestational sac with or without an embryo was associated with 100% specificity and PPV. Conclusion: This study demonstrates that initial TVS is a highly accurate and reliable tool in the evaluation of mothers with clinical suspicion of ectopic pregnancy. With the high accuracy, specificity and predictive values more than 95% of mothers will be evaluated correctly.
