Diabetes mellitus is a metabolic syndrome, prevalence of which is increasing day by day worldwide. The disease is related to a number of micro vascular and macro vascular changes mainly due to hyperglycemia which accelerates the mechanism of deposition of advanced glycation end product in the capillary network leading to hypoxic changes in the tissues resulting in end organ diseases. Diabetic micro vascular complications leads to blindness, end stage renal disease and limb amputations. Top priority treatment of diabetes remains insulin replacement therapy which is till date administered only by parenteral route, which is associated with decreased compliance due to fear of injection and hindrance in daily routine of life. To solve this problem, a number of routes of administrations to name a few nasal, oral and transdermal routes are investigated for long term insulin therapy. Till date no drug has come to the market in its final form. Present study was conducted in three phases that is a survey, study of emulgel in large animal model and in human subjects. Prevalence of diabetic complications in low income group presented in diabetes OPD of Sind Government Hospital, New Karachi Township: Aim of the present survey was to study the prevalence of diabetic micro vascular complications in the low income population group of New Karachi Township. Additionally, what is the impact of metabolic syndrome in the prevalence of complications? To serve the purpose a sample size of one hundred and fifty four participants with type II diabetes for more than 5 years was collected at 80% power and 10% prevalence of diabetes in Pakistan. After acquiring consent from the participants, they were interviewed in detail about their disease, general condition, systemic review, history of co-morbidities, duration of diabetes and medication history. Detailed checkup of feet and eyes by trained personals was carried out followed by blood and urine sample collection for laboratory investigations which included glucose, lipid and kidney profiles. Many parameters were also calculated using different formulas from the data collected by these laboratory investigations. Statistical analysis was carried out by using SPSS -20 (IBM) with Pearson‟s chi squared test for correlation between categorical variables, student‟s t-test for numerical data with level of significance at p < 0.05. Response rate was 50%, and average age of the participants was 50 years. 27% were men and 73% of the participants were women. 84% of the sample belonged to Urdu speaking ethnic group. 68% participants were uneducated or had primary education. Average BMI was 26.28 and average duration of diabetes was 11 years. Majority of patients were found on oral hypoglycemic agents i.e. only 28% participants were prescribed insulin out of which 20% were having insulin along with oral hypoglycemic agents. Most important findings were that 42.89% participants presented with either type of diabetic retinopathy. Proliferative diabetic retinopathy which leads to advanced eye disease however was found 3% (n = 5). 61.69% participants presented with impaired urinary albumin creatinine ratio. 3% (n = 5) of whom presented with overt nephropathy. On the basis of Ipswich touch test, neuropathy was classified in 62% (n = 96) participants, who showed positive loss of sensations on distal phalanges. Metabolic syndrome was found in 60% (n = 93) participants. Analysis of different risk classifications revealed that 8% (n = 13) participants were at high risk to develop diabetic foot (χ2 = 92, p < 0.001),additionally 3 participant presented with active foot. 8% (n = 12) showed high risk to develop coronary artery disease in next 10 years (χ2 = 235, p < 0.001). Risk of stroke was luckily found only 1% (n = 2) in next 10 years. Risk to develop diabetic kidney disease was seen in 8% (n = 13) participants. Gender based stratification showed that average age of female participants was younger (48.5 years) than the male participants (56 years, t = 4.5, p < 0.001). Average BMI of the females (26.99) was greater than the male (24.4) participants (t = 2.88, p < 0.001). There was no gender difference found in the prevalence of microvascular complications i.e. retinopathy, nephropathy and neuropathy. However significant gender difference was found in prevalence of metabolic syndrome as 68% female and 42% male participants presented with the syndrome (χ2 = 9.57, p < 0.025). Significant coronary artery disease risk between gender strata was observed as 64% of male participants had moderate to very high risk to develop coronary artery disease in next ten years compared to females (χ2 = 46.92, p < 0.001). However, risk to develop diabetic foot and diabetic kidney disease was not found statistically different between gender groups. On the basis of metabolic syndrome strata, participants with metabolic syndrome presented with nausea vomiting (χ2 = 6.87, p < 0.001), heavy headedness (χ2 = 13.11, p < 0.001), vertigo (χ2 = 13.4, p < 0.001) and burning sensations of extremities (χ2 = 5.82, p < 0.001). All the microvascular complications were found more prevalent in metabolic syndrome strata i.e. 45% retinopathy, 76% nephropathy and 52% neuropathy. Logistic regression analysis for the complications revealed that body mass index, systolic blood pressure, cholesterol, triglycerides and nephropathy were positive correlates for presence of metabolic syndrome. Retinopathy was found affected by BMI and presence of nephropathy. Glycated hemoglobin, metabolic syndrome and urinary albumin creatinine ratio was found to positively correlate with diabetic nephropathy. Diabetic neuropathy on the other hand was found to be positively correlated with BMI, systolic BP, cholesterol, triglycerides, LDL, HDL, CHR, Serum creatinine, diabetic nephropathy and metabolic syndrome. To conclude, diabetic nephropathy and metabolic syndrome exert their influence in the development and progression of diabetic retinopathy and neuropathy. Study population is at high risk to develop diabetes and its compliations. Hypoglycemic activity of Transdermal dosage form of Insulin in large animal model Aim of this study was to evaluate the effects of optimized insulin emulgel preparation in large animals. Six sheep (Ovis aries) of either sex weighing 20 – 25 kg were used in the study. Baseline sugar level was tested and then anesthesia and hyperglycemia was induced by Ketamine (10 mg/kg) and Xylazine (0.2 mg/kg) for acute and sustained hyperglycemia. Animals were divided into two groups of three animals each. One group received subcutaneous insulin and other received emulgel with the help of a derma roller on shaved skin. The results were found comparable with the insulin injection. Emulgel followed the same pattern of hypoglycemia but a significant difference was observed between the two treatment groups at the end of the study. Hypoglycemic effect of insulin with the help of derma roller was confirmed. Effect of Transdermal insulin on human subjects Objective of this segment of study was to observe the effect of insulin emulgel on human volunteers. 32 human subjects were recruited for the study after sample size calculation at 80% power and 10% prevalence of diabetes in Pakistan. The subjects were divided into healthy and hyperglycemic volunteers. Each group consisted of 16 individuals. After written and informed consent a two day study was conducted. On day 1, baseline data was obtained. Breakfast was provided to the participants to avoid food related bias. On 2nd day participants were applied with emulgel with the help of derma roller then data of blood glucose for next 2.30 hours was collected. In healthy (normoglycemic) subjects good glycemic control was observed, as nonsignificant difference between baseline and post administration data was found. However in hyperglycemic individuals this condition was not found and emulgel was not able to control glucose level after 30 minutes resulting in exaggerated glucose curve.

وہ ترجمان القرآن مولانا حمیدالدین فراہیؒ کے حقیقی پوتے تھے، ان کی تعلیم مدرسۃ الاصلاح کے علاوہ شبلی نیشنل اسکول و کالج میں ہوئی تھی، گھر کے خوش حال تھے، اس لئے کوئی ملازمت کرنے کے بجائے زمین اور زمینداری کی دیکھ بھال کرتے رہے، لیکن قومی و ملی کاموں سے دلچسپی تھی، تقسیم کے زمانہ میں جمعیۃ علماء کانگریس کے پرجوش حامی تھے، تقسیم کے بعد صوبہ کانگریس کے ممبر بھی ہوئے، پھر پرانے کانگریسیوں کی طرح اس سے دل برداشتہ ہوکر لوک دل میں چلے گئے مگر اس سے بھی نباہ نہ کرسکے اور اب سیاسی جھمیلوں سے الگ ہوکر صرف مدرسۃ الاصلاح کی خدمت کے لئے وقف ہوگئے تھے، جس سے اپنے دادا کی یادگار ہونے کی بناء پر ہمیشہ بہت تعلق رکھا، پہلے اس کے نائب ناظم ہوئے اور اب کئی برس سے ناظم ہوگئے تھے۔
وہ جس اخلاص، ایثار و قربانی سے کسی معاوضہ کے بغیر مدرسہ کی خدمت انجام دے رہے تھے، اس کی مثال کسی دینی مدرسہ میں کم ہی ملے گی، ان کے دور میں مدرسہ میں کئی عمارتیں تعمیر ہوئی، بعض مفید کام بھی انجام پائے، یہاں ان کے دادا کی یاد میں دائرہ حمیدیہ قائم ہوا تھا جس کو متحرک بنانے کے بڑے خواہشمند تھے، مرحوم کو دارالمصنفین سے بھی گہرا لگاؤ تھا۔ ان کے دادا مولانا حمیدالدین فراہیؒ کی سرپرستی میں اس کا آغاز ہوا تھا، ان کے بعد ان کے چھوٹے بھائی مولوی حاجی رشید الدین فراہی کو بھی اس ادارہ سے بڑی دلچسپی اور ہمدردی رہی۔

Impact Analysis of Higher Education Research Trajectory on Governance and Public Policy: A Case Study of Four Federal Universities of Pakistan

The research situates link between the graduate researches in four academic institutes of Islamabad and analyzes its impact on the public policy. The study explores the relevance of graduate research trajectory followed by various academic departments of four major federal universities in Pakistan in formulation and execution of public policy. The author collected last five years graduate level dissertation titles from four federal universities and classified them by using thematic approach. An interview guide/questionnaire is prepared accordingly. Chairpersons, deans, professors, bureaucrats, and parliamentarians were approached for semi-structured, in-depth interviews. This is a qualitative study using thematic and analytical approach. The research finds a lack of cohesion between the graduate research trajectory and policy needs of the public institutions. The absence of a national research vision and collaborative framework, difference in training cultures of bureaucracy and academia, lack of logistic support system, defective learning strategies, and an overall neglect for social sciences are the major reasons for incoherence. The analytical correlation of the recorded opinions reveals opportunities to maximize the relevance of the academic research in public policy domain

Effects of 1-Trifluoromethoxyphenyl-3- 1-Propionylpiperidin-4-Yl Urea on Cardiovascular and Metabolic Dysfunctions in Obese and Diabetic Rodent Models

Obesity and diabetes are increasingly important public health problems across the world.Obesity is associated with higher risk for chronic diseases including cardiovascular disease and type 2 diabetes. The increasing trend of diabetes and obesity is imposing a huge burden on health-care systems. Despite therapeutic management with conventional agents obesity and related cardiovascular and metabolic complications remain the major health concerns around the globe. The currently available drugs for obesity and diabetes are associated with multiple adverse effects. Therefore there is an urgent need to develop newer drugs, which are safe and effective. Soluble epoxide hydrolase (sEH) inhibitors offer a novel promising therapeutic target to manage diabetes, obesity and associated complications. Inhibition of soluble epoxide hydrolase increases epoxyeicosatrienoic acids (EETs) levels. EETs are epoxide derivatives formed from arachidonic acid by the action of cytochrome P450 (CYP) epoxygenases specially theCYP2C and CYP2J subfamilies thatmetabolize arachidconic acid to various EETs, such as 5,6-, 8,9-, 11,12-, and 14,15-EETs. EETs are important regulators of the cardiovascular system and have been shown to exert potent vasodilators, anti-inflammatory, antithrombotic, cardio protective and antioxidant effects. Growing number of evidences suggest the over activity of sEH in obesity and diabetes which leads to functional deficit of EETs in the body. Decreased level of EETs is implicated in pathophysiological mechanisms of obesity and diabetes associated complications. Stabilization of EETs through pharmacological inhibition of sEH or sEH deletion has been shown to have beneficial effects in various conditions including pain, inflammation, hypertension and ischemia. Based on diverse physiological role of EETs, the current project was aimed at evaluation of 1-trifluoromethoxyphenyl-3-(1-propionylpiperidine-4-yl) urea (TPPU), a novel soluble epoxide hydrolase inhibitor (sEHI) against fructose induced diabetes and high fat diet–induced obesity in related cardiovascular and metabolic complicationsin rats. TPPUis the most potent soluble epoxide hydrolase inhibitor, metabolically stable and having good pharmacokinetic profile. Although the potential of soluble epoxide hydrolase inhibitor for the treatment of obesity related cardiovascular diseases and metabolic dysfunctions appear promising, it has not been fully established. The current study revealed that rats administered with fructose solution for 12 weeks developed classic symptoms of metabolic syndrome. Controlled rats fed on fructose showed tremendous increase in blood pressure, blood glucose and insulin levels as well as levels of TGs, cholesterol, LDLs were increased (P<0.001). There was significant increase of ALT, ALP, urea and uric acid levels in rats fed on fructose. Endothelium dysfunction was observed in fructose fed animal with attenuated response to acetylcholine.Animals that were fed on fructose and treated with TPPU (2 mg/kg p.o) for 12 weeks improved insulin resistance , decreased blood pressure, blood glucose levels and lowered serum cholesterol, TGs and LDL while HDL levels was elevated, compared to untreated control animals. The fructose induced endothelium dysfunction was also improved with TPPU treatement. The histopathology and immunohistochemistry results of the TPPU treated animals exhibited regenerative effect on ?-cells of the pancreas in diabetic rats. There was increase in the number of islet of langerhans in the pancreas of diabetic rats treated with TPPU compared with untreated rate. The Immunohistochemical staining with anti-insulin antibody confirmed a marked reduction in insulin secreting cells in diabetic untreated rats while TPPU treated group showed normal mass of secreting cells. The results of the present study revealed regeneration in the pancreatic ?-cells in diabetic rats treated with the TPPU (2 mg/kg p.o) for 12 weeks.The present work showed a significant increase in levels of serum liver enzymes (ALP, ALT) in the fructose-fed untreated animals which were significantly decreased in TPPU treated animals. The microscopic examination of the liver sections of diabetic untreated group exhibited great improvement of cellular architecture in the TPPU treated animals. In the present study, obesity was induced rats by feeding them on HFD (Cholesterol 40%, carbohydartes 40% and proteins 20%) for two months. Rats fed on HFD had considerable increase in their body weights compared to age-matched control animals fed on regular diet (Normal control). The increase in body weights of rats fed on HFD and simultaneously administered with TPPU (2 mg/kg p.o) for the two months was significantly decreased (P<0.01). There was also significant (P<0.01) increase in the systolic blood pressure of animals and vascular dysfunction with decreased relaxant response of vessels to acetylcholine and sodium nitroprussidein animals fed on HFD compared to control rats fed on regular diet. This increase in systolic blood pressure and vascular dysfunction was attenuated by TPPU treatment in animals fed on HFD. , In the present study, HDF caused significant increase in plasma creatinine levels (P<0.001) in animals compared to those fed on normal diet. The increased in plasma creatinine levels were inhibited in animals fed on HFD and treated with TPPU for two months. HFD-induced obesity is most commonly associated with systemic inflammation. The present study also investigated systemic inflammation by measurement of circulating obesity markers. The adiponectin plasma levels were highly decreased (P<0.001) in rats fed on HFD for two months compared to control rate fed on normal diet (p < 0.001).Adiponectin levels were increased in rats fed on HFD and treated with TPPU. Leptin resistance was also increased in obese animals associated with higher leptin levels inanimals fed on HFD compared to those fed on normal diet. The increase in leptin levels was significantly decreased in animals fed on HFD and treated with TPPU for two months.In the present investigation obese animals had increased levels of plasma insulin compared to the control animals fed on regular diet (normal control). This increase in insulin levels was significantly suppressed (P< 0.001) by TPPU treatment in rats fed on HFD. The other metabolic parameters such as plasma cholesterol, TGs, LDLs were increased and HDL decreased in obese animals fed on HFD compared to those fed a regular diet.HFD also caused tremendous increase in serum ALT and ALP. The increase in lipid levels was inhibited and HDL levels enhanced in animals fed on HFD and treated with TPPU. TPPU treatment also reversed HFD induced ALT and ALP abnormalities in rats caused by HFD treatment. The decrease in liver enzymes was also reflected in the finding from microscopic examination of the liver sections showing marked improvement in the liver architecture in animals fed on HFD and treated with TPPU compared to those treated with vehicle and fed on HFD. These findings indicate that TPPU could be potential new drug to manage obesity induced cardiovascular and metabolic complications. These findings indicate that therapeutic and/or preventive potentials of TPPU against fructose induced diabetes and HFD-induced obesity and the related cardiovascular and metabolic complications in rats.
