Comparison of Outcomes of Open Reduction Versus Open Reduction Salter Osteotomy for Developmental Dysplasia of the Hip Ddh Among Children at French Medical Institute for Children, Kabul, Afghanistan
Background: Developmental Dysplasia of the Hip (DDH) consists of either subluxation or dislocation of femoral head from acetabulum and acetabular dysplasia. The ratio of left side hip to right is 3:1. There are several kinds of treatments for DDH globally, and conservative treatment is one of the best methods with a better outcome than surgical treatment. Objective: The aim of this research study is to compare the outcomes of Open Reduction versus Open Reduction Salter Osteotomy with regard to length of stay, complications and family satisfaction among children with DDH at FMIC, Kabul, Afghanistan. Methodology: A retrospective cohort study design was employed. Records of 166 participants who underwent Open Reduction and Open reduction Salter Osteotomy at FMIC hospital were reviewed. The data was collected from the available records during the period of 2010 to 2015. Chi-square test of independence and T-test for two independent samples were run to determine an association between two categorical variables and compute mean difference between categorical and continuous variables for two types of surgical procedure for DDH respectively. In addition, t-test for two dependent samples was also run to compare the mean difference of Pre-operation and Post-operation AI. All the ethical principles were followed throughout the study process. Result: The research findings revealed that there is a difference in outcomes of Open Reduction versus Open Reduction Salter Osteotomy. There was a meaningful difference of Pre-operation and Post-Operation AI for DDH patients (p-value <0.001). Hospital length of stay for Open Reduction group was relatively low (p-value <0.001). The rate of development of complications was higher in Open reduction group as compared to Open reduction Salter osteotomy. Residual dysplasia is more common in open reduction group, 16 participants (19.27%) as compared to open reduction salter osteotomy 9 (10. 84%). In open reduction group only 1 (0.6%) of the participants had subluxation, and 2 (1.2%) others developed femoral head AVN. On the contrary, in the open reduction salter osteotomy group, 1 (0.6%) participant had developed subluxation and 5 (3%) had developed AVN and majority of participants 132(79.5%) had not developed any complications. Half of participants’ families 101 (60.8%) were strongly satisfied, followed by 62 (37.3%) satisfied, and while 3 (1.8%) were dissatisfied with two types of surgical treatment methods. 132 (79.5%) of participants had excellent outcome, 25 (15.1%) good, 2(1.2%) and 7 (4.2%) poor outcome. This study aims to determine the implementation of special character education on the impact for students and school components at Madrasah Aliyah Negeri (MAN) Insan Cendekia Gorontalo. This research method is a qualitative field research. Data collection was carried out through preliminary or pre-observational studies, observations, and interviews. The characters developed in this madrasa include disciplined, religious, polite, responsible, hard work and cooperation. In other words, this character development is carried out in the madrasah program of activities both through learning, as well as extra ¬ curricular as well as religious and self-development activities. The impact of the results of the implementation of character education at MAN Insan Cendekia Gorontalo has a positive impact for all groups, especially for students and the school community. The implementation of character education at MAN Insan Cendekia Gorontalo is very beneficial for students, due to changes in attitudes that are even better and according to the expectations of parents, where their children live disciplined, productive, independent and able to practice religious teachings.
There is a good deal of consensus in literature about the significance of the role of the head teacher in schools. Their role is considered as crucial for school effectiveness. This research aimed to explore the role of the head teachers in the teaching and learning process of the school. This study was conducted with three head teachers belonging to different education sectors of Pakistan included government, private for and not-for-profit. The research studied the roles perceived and enacted by the head teachers with a focus on their instructional role. It also investigated the perceptions and understanding of the head teachers about enquiry instructional strategy and the role they played in implementing this strategy in the classroom. In order to achieve the purpose, a qualitative enquiry approach was adopted. The research drew on qualitative data from methods such as conversation, shadowing, document analysis, and reflective journal of the researcher. Role of head teachers was studied under two important dimensions: leadership role which is broader in scope and instructional role which is the specific component of leadership role. Three categories captured head teachers' conceptualization and practices of their instructional role: administration of instructional activities, curriculum and classroom instructions, and professional development of teachers. The study revealed that head teachers give their excessive attention to the administration of instructional activities, and less time was spent to perform other two categories of instructional role. This enactment was influenced by certain factors included their own perceptions of their roles, their school structure, their autonomy and their exposure to the professional development programs. The research also showed that the head teachers' understanding about the enquiry instructional strategy was restricted to the knowledge dimension of the same. Other aspects as skills and dispositions were not included in their conceptualization. The head teachers also shared their perceptions of their role in the implementation process. Their perception of their roles was limited to the administrative component of their instructional role ignoring the complex nature of it. Recommendations and suggestions of the research for future study are also pointed out.
