Objective: This was a prospective analytic study that aimed to compare the agreement between CT pulmonary angiography (CTPA/CTV) and ultrasonography (US) for diagnosis of thromboembolic disease and determine the sensitivity, specificity, positive and negative predictive values of the 2 tests using the presence of thromboembolic disease as a reference standard. Methods: 76 consecutive patients who were referred for CTPA at the Radiology department of Aga Khan University Hospital over an 8 month period between December 2008 and July 2009 were included in the study. All the patients underwent combined CTPA/CTV according to the usual departmental protocol. They subsequently had bilateral lower limb duplex US within 24 hours of the CTPA/CTV study. Results for the presence or absence of DVT were analyzed for both CTV and US. Presence of thromboembolic disease was defined by a composite reference standard and this was used to determine the sensitivity, specificity, positive and negative predictive values for both tests. Results: 73 patients were included in the final analysis. The 2 imaging tests had similar results in 62 (84.93%) patients; CTV was positive and US was negative in 7 (9.6%) patients, while CTV was negative and US was positive in 4 (5.5%) patients. There was a moderate level of agreement (k= 0.528) between CTV and US for detection of DVT. CTV had a much higher sensitivity (94.1%), specificity (100%), PPV (100%), NPV (98.2%) compared to US (58.9%, 94.6%, 76.9%, 88.3% respectively) when the presence of thromboembolic disease was used as a reference standard. Conclusion: CTV has higher sensitivity, specificity, positive and negative predictive values for detection of asymptomatic DVT compared to US when the presence of venous thromboembolic disease is used as a reference standard. Therefore, in patients suspected to have PE, CTV can be used to detect DVT without an additional duplex US examination.

اس کائنات ِرنگ و بو میں ہر چیز اپنی شناخت اور پہچان رکھتی ہے، اپنے وجود کے اظہار کے لیے اس کی کوئی نہ کوئی شناخت ہے سورج کی شناخت یہ ہے کہ وہ مشرق سے نکلتا ہے اور مغرب میں غروب ہو جاتا ہے، ستاروں کی شناخت یہ ہے کہ وہ رات کو آسمان پر درخشاں ہوتے ہیں۔ پہاڑوں کی شناخت اور پہچان یہ ہے کہ دور سے دکھائی دیتے ہیں اور بلندو بالا ہوتے ہیں۔ اسی طرح پھول کی پہچان ہے کہ وہ خوشبوفراہم کرتا ہے اور اپنے اردگرد کے ماحول کو معطر اور خوشبودار رکھتا ہے۔ اسی طرح مسلمان کی بھی ایک پہچان ہے وہ یہ کہ اس کے ہاتھ اور زبان سے دیگر مسلمان محفوظ رہتے ہیں۔ وہ کسی کو گزند نہیں پہنچا تا کسی کو تکلیف پہنچانا اس کو ناگوار گزرتا ہے۔ کسی کی پریشانی وہ اپنی پریشانی سمجھتا ہے۔ تو بس اس بات کا قائل ہوتا ہے کہ آدمی اور انسان کا احترام کیا جائے اور اس کے حفظ مراتب کا خیال رکھا جائے۔ قرآنِ پاک میں بھی انسان کے ساتھ حسنِ سلوک اور رواداری کی تلقین آئی ہے۔ ارشادِ باری تعالیٰ ہے:
’’لوگوں کے ساتھ احسن طریقے سے گفتگو کر یں، حسن سلوک سے پیش آئیں ‘‘

In this research I talk about the coordination in Holy Qur’ān  Chapters, so as to each Chapters contains a specific purpose that its Qur’ān ic verses want to achieve it, and we don’t see any difference or inconsistency. In order to achieve this purpose. I make an analytic study for one chapter in holy Qur’ān.I gathered the declarations of the explainers of, after that I give all my effort to show the coordination between them.

Analysis and Quantification of Medicinally Important Essential Oil by Mass Spectrometry Method and Chemical Studies on Opuntia Dillenii

The thesis comprises two parts, A and B. Part A deals with the analysis and quantification of medicinally important essential oil, by mass spectrometry method while, the part B describes the chemical studies on cladode of Opuntia dillenii. A short summary on the biosynthesis of steroids, flavonoids, fatty acids and terpenoids are also included in the thesis. Part A This part explains the chemical constituents and their quantification of medicinally important components of extracts on Ixora fulgens, I. coccinea (yellow and orange color flowers), I. polyantha, I. chinensis, Ipomoea batata (blackie), I. batata pink frost, and Cassia fistula, by mass spectrometry method. GC and GC-MS studies were carried out on extracts of different parts especially flowers, leaves and stalks of the plants which led to the identification of 230 chemical compounds from hexane extracts, 324 chemical compounds were identified from chloroform extract of Ixora species. Moreover, 26 metabolites were characterized from methanol extracts of Ixora chinensis flowers extract. From Ipomoea species, 106 chemical compounds were identified from hexane extracts, while, 72 compounds were identified from chloroform extract of the plant. Moreover, 57 phytochemicals were identified from hexane extract, whereas, 51 metabolites were identified from chloroform extracts of C. fistula plant. Different class of compounds were identified from this extracts including hydrocarbons, long chain fatty acids and their esters, alcohols, aromatic acids and their esters, terpenes and vitamins. Antibacterial, antifungal, antioxidant, anti-inflammatory, and anti-cancer activities have also been carried out from all the above mentioned extracts. Part B Total thirteen different fractions of vaccum liquid chromatography (VLC) of antidepressant butanol phase of Opuntia dillenii were selected for chemical analysis. For this purpose GC and GC-MS studies were carried out, which led to the identification of 409 chemical compounds.
