Background: Diagnosis of extrapulmonary tuberculosis (EPTB) continues to be a challenge due to the complexity of the causative organism and the wide array of pathologic features seen in this infection. A number of studies have shown polymerase chain reaction (PCR) for EPTB to be a feasible, sensitive and specific test for diagnosis of tuberculosis (TB) of which, the World Health Organisation (WHO) has given recommendations. As pertains EPTB, Xpert MTB/RIF may be used in place of conventional tests such as microscopy, histopathology and culture in lymph nodes and other tissues from patients suspected to have EPTB. It has been demonstrated that Xpert MTB/RIF can be used for fresh or frozen tissues specimens with good results. However, the use of formalin fixed paraffin embedded (FFPE) tissues, a widely available and rich source of clinical material, on the Xpert platform is yet to be described. Objectives: The aim of this study was to demonstrate the potential utility of Xpert MTB/RIF in the detection of Mycobacterium tuberculosis from FFPE tissues with histological features suggestive of tuberculosis. We compared Xpert MTB/RIF to histology for the detection of Mycobacterium tuberculosis from FFPE tissues. In those cases with a positive Xpert result, we determined the prevalence of rifampicin resistance in EPTB. Methods: Eighty randomly selected archived FFPE tissues exhibiting histological features suggestive of TB (necrotizing granulomatous inflammation, non-necrotizing granulomatous inflammation, chronic inflammation, necrotizing inflammation and suppurative inflammation) from January 1, though December 31, 2014 were retrieved. All the cases were subjected to microscopic evaluation of Haematoxylin and Eosin (H&E) stained and Ziehl-Neelsen (ZN) slides. With prior deparaffinization and lysis, all cases were also subjected to the Xpert MTB/RIF assay. Only 79 cases were included in the final analysis. This was after exclusion of one case due to an error in the Xpert MTB/RIF assay. The outcome measures were proportions of positively identified cases by each test. The data were analysed using chi - square test. Results: Xpert MTB/RIF assay detected 32.9% more cases than microscopy (53.2% versus 20.3%) which was statistically significant, (p= 0.002). None of the cases tested positive for rifampicin resistance. Seven cases however, had an indeterminate rifampicin resistance result. Conclusion: With prior deparaffinization and lysis, FFPE tissues can also be subjected to the Xpert MTB/RIF assay. A validation study to determine the clinical utility of this assay for FFPE tissues is recommended.

اس میں کوئی شک نہیں کہ معاشروں کے اتار چڑھاؤ میں حقوق العباد کا بہت زیادہ عمل دخل ہے ، اگر آپس کے حقوق و فرائض افراد پورے کرتے رہیں تو کوئی بگاڑ اور فساد پیدا ہی نہ ہو۔فساد اسی وقت پیدا ہوتا ہے جب کسی کے حقوق دبائے جائیں ۔ اسی وجہ سے قرآن وحدیث میں بڑی تاکید کے ساتھ حقوق العباد کی ادائیگی کا حکم دیا گیا ہے ۔
اسلامی ریاست میں تمام شہری حقوق و فرائض کی ادائیگی میں برابر ہیں۔حقوق و فرائض لازم و ملزوم ہیں۔ایک شخص کے جوفرائض ہوتے ہیں ، وہی دوسرے کے حقوق ہوتے ہیں، جیسا کہ شادی شدہ عورت کو نان و نفقہ ، لباس اور رہائش دینا اس کے خاوند کے ذمہ ہے اور یہ عورت کا حق ہے۔ اس کے بدلے میں عورت پر یہ فرض عائد ہوتا ہے کہ وہ اپنی عزت وناموس کی حفاظت کا خیال رکھے اور بچوں کی تعلیم وتربیت پر خصوصی توجہ دے ۔ بالکل اسی طرح ریاست اور اس کے افراد کے بھی آپس میں حقوق و فرئض متعین ہیں ۔ ریاست ٹیکس وصول کرکے اور قوانین وآئین پر عمل پیراکروا کے معاشرے کو لوگوں کو تعلیم ، صحت اور آزادی کی سہولت مہیا کرتی ہے۔ ا سلام کا یہ خاصہ ہے کہ اس نے فرائض کی ادائیگی پر زور دیا ہے اور اسی کے ضمن میں حقوق خودبخود آجا تے ہیں۔دینِ اسلام چونکہ عدل و انصاف اور مساوات کا دین ہے، اس لیے اسلامی ریاست میں تمام شہریوں کو سماجی، سیاسی اور اقتصادی حقوق برابری کی بنیاد پر حاصل ہوتے ہیں۔ اسلامی ریاست ملک سے جرائم کا خاتمہ کر کے شہریوں کو عدل و انصاف فراہم کرتی ہے ، ہر فرد کی جان و مال اور عزت وآبرو...

Islamic literary legacy is diverse and multidimensional in Sub-continent despite its being prone to religious b and the issue of migration integral part of Islamic literary legacy is the Quranic translations & interpretations. The main aspect of these translations & interpretations are the endeavors put forward by Muslim as well as non-Muslim scholars. Keeping in view the endeavors translations & interpretations of Quran, the non-Muslims minorities of sub-continent can be divided into two groups. The first group of part is based on the followers of Judaism, Parsee, Buddhist, Sikhism, Jainism and idols of Kalash whose translations and interpretations is not well known. Whereas, the second group endure Christians, Hindus and Qadyanis. Christians and Hidus consider Quran as non-revealed. Since the style of their interpretations is based on criticism and rejection in aggressive manner. Whereas the interpretations by Qadyanis are focused on religious polarized beliefs and ideologies taking into considerations Quran as revealed. That’s why Qadyanis interpretations are not accepted in Muslim community. The criticism by non-Muslims writers has resulted in rational approach for study of Quran instead of tendency based on esteem among Muslim. This paper represents aforementioned non-Muslims services regarding Quranic Interpretations and Translations which are distinguished in Muslims Interpreters.

Fem Based Numerical Solutions of Incompressible Flows in a Finned Double-Pipe

The aim of the present work is to numerically simulate laminar forced convection in the fully developed flow through the finned annulus of a double-pipe subject to constant heat flux boundary condition (H1) and to investigate the effects of various geometric designs and arrangements of fins on the thermal performance of the finned duct. The designs of the finned annulus considered in the present work are longitudinal triangular fins with equal and unequal heights, and fins with variable fin-tip thickness. The governing partial differential equations of the convection problems are numerically solved by employing the finite element method (FEM) and the discontinuous Galerkin finite element method (DG-FEM). The hydraulic and thermal characteristics like the friction factor, the Nusselt number and the surface flow area goodness factor are studied against various geometric design parameters. Moreover, the validity of various assumptions in simpler models is investigated in more realistic models. The behaviour of the flow and thermal characteristics has been found to match with the physics of the problems and therefore, substantially validates the present simulations. For highly conductive triangular fins of equal heights, the results show upto more than 4 i times gain in the heat transfer rate relative to increase in the pressure loss because of the attachment of fins when the equivalent diameter is used as the characteristic length. Using fins in two groups of different heights, the velocity and temperature distributions can be significantly altered to have more favourable flow and thermal characteristics. For specified values of the number of fins, the thermal conductivity ratio and the size of the inner pipe, a combination of unequal heights of the two fin groups rendering maximum heat transfer coefficient exists in many cases. Such combinations of fin heights are very useful in reducing pressure loss and promoting heat transfer rate. The present study recommends the use of unequal fin heights depending on the cost, weight and heat duty requirements. For longitudinal fins with specified base angle and variable tip thickness, no single tip-angle renders maximum values of the heat transfer coefficient and surface flow area goodness factor for all values of the height and number of fins indicating that the choice of fin-tip angle strongly depends on the number of fins and their height. It is found that highly significant gain in the Nusselt number can be achieved by considering variation in the thickness of fin tip for various values of the conductivity parameter. This gain for trapezoidal fin is upto 38% over the triangular fin and 163% over the rectangular fin. Therefore, the consideration of the fin-tip thickness as a design parameter is significantly important for the optimal design of the present heat exchange system. All the computed results are in excellent comparison with the available literature results.
