Background: Laryngeal mask airways (LMAs) are widely used in anaesthesia and are considered to be generally safe. Postoperative sore throat (POST) is a frequent complication following LMA use and can be very distressing to patients. The use of an LMA cuff pressure of between 30 and 32cm of H20 in alleviating postoperative sore throat has not been investigated. Primary objective: To compare the occurrence of POST between the intervention group in which LMA cuff pressures will be adjusted to 30-32cm of H20 and the control group in which only monitoring of LMA cuff pressures will be done. Secondary Objectives: To compare the severity of POST between the two study groups. To compare the LMA cuff pressures between the two study groups. Study Design: A single blind randomized control trial Methods: Eighty consenting adult patients scheduled to receive general anaesthesia with use of an LMA were randomized into two groups of 40 patients each. In the intervention group LMA airway cuff pressures were adjusted by the principal investigator to a pressure of 30 to 32cm of H20.The control group only had LMA cuff pressures monitored throughout the surgery. All patients were interviewed postoperatively at two, six and twelve hours. Data of their baseline characteristics, occurrence and severity of POST was collected. If POST was present; a Numerical Rating Scale (NRS) was used to assess the severity. Cuff pressures between the two study groups were also determined. Results: The baseline demographic characteristics of the participants were similar. The use of manometry to limit LMA AMBU® AuraOnce™ intracuff pressure to 30-32cm H2O reduced POST in surgical patient’s by 62% at 2 hours and 6 hours (Risk Ratio 0.38 95%CI 0.21-0.69)in the intervention group. The median POST pain score in the intervention group was significantly lower than the control group with scores of 0 at 2, 6 and 12 hours post operatively. Routine practice of LMA cuff inflation by anesthesiologists is variable, and the intracuff pressures in the control group were higher than in the intervention group. (P<0.001) Conclusion: Among this population, reduction of LMA AMBU® AuraOnce™ intracuff pressure to 30-32cm H2O reduces the occurrence and severity of POST. The LMA cuff pressures should be measured routinely using manometry and reducing the intracuff pressures to 30-32 cm of H20 recommended as best practice.

IMPLEMENTASI STRATEGI HAFALAN AL-QUR'AN TERHADAP HASIL BELAJAR SISWA DI MADRASAH ALIYAH AL-HUDA KOTA GORONTALO

  Abstrak Artikel ini bertujuan untuk mengungkapkan Implementasi Hafalan Al-Qur’an terhadap Hasil Belajar Siswa di Madrasah Aliyah Al-Huda Kota Gorontalo bahwasanya mengikuti program tahfiz peserta didik senantiasa terlatih untuk mempermantap bacaan Al-Qur’an mengetahui penerapan ilmu tajwid yang baik dan benar. Strategi implementasi hafalan Al-Qur’an bagi peserta didik ialah untuk menuntun siswa agar mudah dalam mengahafal Al-Qur’an, peserta didik yang ikut serta dalam program tahfiz rata-rata memiliki kemampuan yang baik dalam membaca Al-Qur’an yang baik dan benar berdasarkan kaidah tajwid, serta memiliki hafalan ayat-ayat Al-Qur’an yang nantinya menjadi bekal dasar untuk memperdalam ilmu pengetahuan agama. Implikasi penelitian ini adalah guru tahfiz di Madrasah Aliyah Al-Huda Kota Gorontalo terus meningkatkan sumber daya dalam strategi hafalan Al-Qur’an untuk menunjang proses kegiatan pembelajaran khususnya mata pelajaran yang membidangi Al-Qur’an seperti Al-Qur’an Hadist, Ilmu Al-Qur’an Tajwid dan Tahfiz. Kepada semua komponen yang ada di Madrasah Aliyah Al-Huda Kota Gorontalo yakni pihak yayasan, komite, orang tua dan kepala sekolah, diharapkan dapat bekerjasama untuk kelancaran program pendidikan terutama pada program tahfiz Al-Qur’an. Molecular Characterization of Microbial Oxidoreductases and Their Application in Polymer Biodegradation

The detoxification of xenobiotic organic compounds by various microorganism through oxidative coupling is facilitated with oxidoreductases. With the help of energy yielding biochemical reactions these microbes extract energy for their metabolic pathway. They promote the transfer of electrons from a reduced organic substrate to another chemical compound. Enzymatic bioremediation of toxic organic pollutant is the very effective strategy in complex environmental conditions. Oxidoreductases enzyme have a significant potential for the bioremediation of the xenobiotic compounds. Various electron donor complex polymeric substrates containing Phenol and aromatic amines oxidized by peroxidase in the presence of H2O2 while O2 in the case of dioxygenase. Fungi are important among microorganisms for production of extracellular peroxidases. The present study aimed for production and characterization of peroxidases (laccases and lignin peroxidases) and dioxygenase for polymer biodegradation. Fungal strains named as Nk-1, Nk-2, Nk-3 and Nk-4 were selected and processed for different optimization studies. NK- 1 was identified 18s rRNA sequencing as Phanerocheate chrysosporium was further selected for enzyme production. Maximum enzyme production was observed at temperature at 30°C, pH 5.5. Statistical analysis indicates the significance of model and components based on F value and P value <0.05. Purification of enzymes was done by column chromatography. The molecular weight estimation was carried out by Sodium dodecyl sulphate SDS. Fourier transform infrared (FTIR) spectroscopy of enzyme treated substrate revealed the structural changes as compared to control (without enzyme treatment). The decrease in weight (0.08g) was observed in enzymes treated film then control (0.1g). FTIR spectroscopy of enzyme treated plastic film revealed the structural changes as compared to control (without enzyme treatment) i.e. a new peak appeared at 3367 cm -1 (Alkenyl C-H stretch). The peak at wavelength 1633 cm-1 (C=C stretching vibration of aromatic ring) in treated sample was sharp and larger as compared to the control. A visible change in surface of PVC films in term of surface erosion was observed by Scanning Electron Microscopy (SEM) after enzyme treatment. Peroxidase enzymes have wide range of applications from biodegradation of recalcitrant compounds to paper and pulp, cosmetic, food and textile industry. Enhanced production from food waste as cheap substrate was studied. Biosynthesis of lignin peroxidase, manganese peroxidase and laccase enzymes from food waste as a substrate is possible and their production can be enhanced by optimizing different factors. The native way of synthesizing dioxygenase, laccase and lignin peroxidase are not enough to the current industrial demands mostly because of their small amount of yield and the high cost of purification or synthesis. Heterologous expression may allow the production of laccase and lignin peroxidase in higher amount and with desired characteristics. In the present research study, the heterologous expression of fungal lignin peroxidase and laccase were performed in the cells of DHα5 strain of E. coli bacteria. With the help of shake flask experiment, the enzyme assay for enzymes was performed and a variation in the enzyme activity was observed during incubation time. Molecular weight of lignin peroxidase and laccase were found to be 46 kDa and 66 kDa respectively, as determined by the SDS-PAGE analysis. The enhanced production of fungal lignin peroxidase and laccase can be achieved through heterologous expression in bacterial host so that the biodegradation of recalcitrant plastic waste can be speed up.
