Background: Systemic arterial hypertension is the main modifiable risk factor for development of cardiovascular disease, which is the number one cause of death in high income countries and is increasingly becoming a cause of concern in Sub-Saharan Africa (1). Awareness, management and overall control of hypertension in Sub-Saharan Africa is poor (2,3). Poor control stems from various causes; one being patient non-adherence with medication, a factor amenable to modification. The use of mobile health for behavioural change from low resource settings is gaining widespread use in management of communicable diseases (4,6). Mobile phone has the potential to significantly influence patients to improve adherence to antihypertensive medication thereby leading to better control of hypertension and forestalling complications (6). In Kenya, there is, to date, no evidence of a trial on mobile text messaging to determine whether there is any change in adherence rates to antihypertensive therapy in patients. Objective: The primary objective is to set-up and carry out a Hypertension Short Message Service system to assess whether sending weekly reminders as text messages via a mobile phone will improve adherence rates to antihypertensive treatment, which has been found to translate to improved blood pressure control. Methods: The study was a Randomised Controlled Trial with an intervention arm and control arm involving hypertensive patients seeking treatment at the Family Medicine Clinic at Aga Khan University Hospital, Nairobi. Results: Prior to carrying out the Hypertension Short Message Service system the level of adherence to antihypertensive therapy in the intervention group was 60.3% compared to an adherence rate of 61% in the control group. Six weeks following the intervention, the adherence rate increased by12.4% and 5.3% in the intervention and control groups respectively. The intervention had a statistically significant effect represented by a 7.1% improvement in adherence rate in intervention arm above the control arm (p < 0.001). Diastolic blood pressure measurements reduced from an average of 87.9 mmHg to 81.8 mmHg in the intervention group compared to a reduction from 86.8 mmHg to 82 mmHg among patients in the control group. Systolic blood pressure measurements reduced from an average of 141.4 mmHg to 131.9 mmHg in the intervention group, while there was an increase in the control group from 140.4 mm Hg to 142.5 mmHg. Following the Hypertension Short Message Service intervention, the difference in diastolic blood pressure in the intervention and control arm was 1.24mmHg, a reduction that was not statistically significant

Education and practical orientation are both significantly interlinked. Education without practical training stands mere theory having no connection with life. Practical orientation without education carries no meaning. Islamic system of life seeks to espouse both education and training. The Prophet (s. a. w.) educated his people theoretically as well as trained them practically. What ever he did in this regard constitute Islamic principles for human life. Curricula of educational institutions need to be developed with a view to making students knowledgeable on the one hand and upholders ofIslamic virtues in their practical life on the other. The present paper is aimed at highlighting the co-relationship between the education and practical orientation, proving beyond doubt that isolation of one from the other is too damaging to imagine

The Effect of Aga Khan University’S Examination Board on School Curriculum of a Private School in Karachi, Pakistan: A Case Study

Research shows that assessment plays a pivotal role in the teaching learning process. While some forms of assessment enhance teaching learning, others may inhibit it. Focusing on assessments, Aga Khan University Examination Board (AKU-EB) was established in 2003 to offer SSC certificate at the end of secondary schooling under the National Curriculum. Its objective was to restore the deteriorating standard of SSC certificate offered by schools affiliated with the Public Examination Boards. Many of the schools affiliated with the Public Board have shifted to the AKU-EB board after its inception. This study was conducted in order to seek to understand how changing from the Public Board to AKU-EB effects the curriculum of a private school in Karachi, Pakistan. In order to achieve this purpose, a case study approach in the qualitative paradigm was adopted. The research drew on semi-structured interviews of various stakeholders, including the Principal, teachers and students, classroom observations and document analysis as the sources of data collection. The study findings revealed that the AKU-EB examination assesses the students, on the basis of the student learning outcomes (SLOs) of the National Curriculum at three cognitive levels - knowledge, understanding and application - rather than a prescribed textbook. This has been a major driving force for change. The teachers refer to multiple sources of information and the students are all familiar with the SLOs as prescribed in the examination syllabus. The study showed that there has been a major impact on the school curriculum by the change in the examination methodology. These effects, among others, include a paradigm shift in the teaching approach from traditional teaching to student-centered learning, opportunities for subject content integration, and students moving towards becoming independent self- learners. The teachers have moved towards collaborative lesson planning, and there is development of a positive teacher-student relationship. The study also showed that there has been a ripple effect on the lower secondary school curriculum. In the light of these findings, the study also makes recommendations and suggestions for future research.
