Background: As the prevalence of cardiovascular diseases in Sub-Saharan Africa rises, coronary artery disease, with its acute presentations, is being increasingly recognized in Kenyans and treated at tertiary level hospitals. It is just over a decade since the introduction and wide availability of modern cardiology services, including interventional cardiology in Kenya. Following an acute coronary syndrome (ACS), patients remain at high risk of death and other adverse events such as heart failure, recurrent myocardial infarction, stroke, and bleeding. Local and regional data on long-term outcomes following ACS are lacking. These data are important to clinicians for prognostication and to health care planners for resources allocation. Objectives: The objectives of this study were to determine the in-hospital and long-term outcomes of patients following the treatment of Acute Coronary Syndromes. Specific objectives included determining in-hospital, 30-day and one-year mortality of ACS patients, and the rates of several non-fatal adverse outcomes including reinfarction, heart failure and cardiogenic shock, revascularization, stroke, major bleeding and re-hospitalization due to specific major adverse events. Methods: The study was a retrospective chart review of ACS admissions during a two year period (2012-2013) for all acute coronary syndrome admissions. Data on patient characteristics, treatment, and inpatient and short-term outcomes were obtained from the patients’ medical records. Telephone interviews were conducted to determine long-term results. Results: A total of 230 patients were included in the analysis. Of these, 101 had a diagnosis of STEMI, 93 suffered an NSTEMI, and 36 had UA. Males accounted for 81.7% of the patients, and the mean age was 60.5 years. Delayed presentation was common with more than 35% of patients taking longer than 24 hours to arrive. Coronary angiography was performed in 85.2% of the patients. The in-hospital mortality was 7.8% (14.9% for STEMI and 2.3% for NSTE-ACS), the mortality at 30 days and one year was 7.8% and 13.9% respectively. The most common in-hospital non-fatal adverse outcome was heart failure, occurring in 40.4% of STEMI and 16.3% of NSTE-ACS. Readmission rate due to recurrent MI, stroke or bleeding at one year was 6.6%. Conclusion: In our cohort, in-hospital, 30 day and one year mortality following acute coronary syndromes remains high, particularly for STEMI. Delayed presentation to hospital following the onset of symptoms appears to be an important contributing factor.

کائنات میں اشرف المخلوقات ہونے کا شرف صرف انسان کو حاصل ہے تمام مخلوقات میں انسان سب سے زیادہ با اختیار بھی ہے۔احساسات و جذبات بھی سب سے زیادہ انسان ہی رکھتا ہے۔اسی لیے وہ ازل سے اپنے خیالات کا اظہار کرتا آیا ہے۔کبھی آرٹ ،تصاویر،لوک داستانیں اور کبھی شعر وادب کی صورت زبان کا اظہار بہترین ذریعہ ہے۔لفظوں کو ایک خاص انداز میں اپنے خیالات کے اظہار لے لیے استعمال کرنا شاعری ہے۔راکی ولسن نے بھی اپنے خیالات کے اظہار کے لیے خوبصورت الفاظ کا استعمال کیا ہے۔اس لیے اسکی شاعری و خود جذبات کے تحت وجود میں آتی ہے۔

راکی نے رشتو ں ناتوں میں بٹے ہوئے انسان کو اس کی خوشیوں،غموں،امنگوں، آرزوؤں،اْمیدوں،نا امیدوں،رویوں اور ولولوں کو اپنا موضوع بنایا ہے۔اسکی بے ساختہ شاعری فطرت انسانی کی عکاس ہے،وہ موضوعاتی شاعری پر بھی طبع آزمائی کرتے دکھائی دیتے ہیں۔انکی شاعری میں متاثر کرنے کی زبردست صلاحیت موجود ہے۔یہ صلاحیت صدق دل اور خلوص نیت کے بغیر حاصل نہیں ہوتی۔یہ خلوص نیت ان کے مجموعہ کلام ’’مینار محبت‘‘ میں محبت بانٹتے ہوئے دکھائی دیتا ہے۔آپ کے اس مجموعہ کلام اجمالی جائزہ کچھ اس طرح سے ہے۔

اگرچہ شاعر نے شاعری کی روایت سے ہٹ کر اس کتا کو شروع کیا ہے۔مگر اس کے باوجود وہ اپنے پروردگار کو بھولے نہیں۔کہتے ہیں کہ ہر شے میں اس ذات باری تعالیٰ کا جلوہ موجود ہے اسی لیے تو کائنات کی ہر شے اس کی خوبصورتی کو دیکھنا چاہتی ہے،آج راکی صاحب جو کچھ بھی ہیں وہ سب اس ذات کی کرم نوازی کی وجہ سے ممکن ہوا ہے:

Law plays a pivotal role in the establishment of any peaceful society. The Relationship Between Occupational Stress and Organizational Commitment in Non-Governmental Organizations of Pakistan

The purpose of this research was to extend the existing body of knowledge on the relationship between occupational stress and employees’ organizational commitment into the context of non-governmental organizations of Pakistan, as little research have been conducted on this relationship in Pakistan. It was a descriptive correlational study in which scientific methodology was used to answer the research questions. Based upon the review of the recent literature available on the subject, a conceptual framework was developed to study the relationship between occupational stress and organizational commitment in the NGO sector in Pakistan. The framework illustrates three dimensions of occupational stress including occupational role stress, personal strain and coping resources and their hypothesized relationship with organizational commitment, as well as the influence of some major demographic factors including age, experience, job position, gender, qualification, income and marital status on the relationship between occupational stress and organizational commitment. Occupational stress inventory-Revised Edition and Organizational Commitment Questionnaire were used to collect data. The overall response rate from the NGO employees was 64%. Several statistical techniques, including arithmetic mean, standard deviation, correlation analysis, percentile analysis and analysis of variance-ANOVA were used to test the hypotheses of the study. Consistent with prior research, an inverse relationship was found between occupational stress and organizational commitment. The level of occupational stress experienced by NGO employees was found to be high. The occupational roles that employees play were reported as the main source of stress. The level of employees’ organizational commitment was generally moderate but with a tendency towards high. Normative commitment was reported as the strongest form of commitment. Demographic variables of age, experience, job position and income as well as gender significantly influenced the relationship between occupational stress and organizational commitment. This study is significant because of the insights it provides for the NGO managers to better understand the occupational stressors inherent in the functions of their workforce. Furthermore, this study develops an understanding of the commitment process and its relationship with occupational stress to promote commitment among employees and help them achieve their organizational goals effectively. NGO management can utilize the research findings to formulate appropriate strategies to address the stress related problems of their employees.
