Background: Breast cancer is an emerging health concern in Africa. It is the most common female cancer in Kenya. The accuracy of the diagnostic modalities in our institution has not been previously assessed, especially in cases where components of the Triple Test (TT) are non-concordant. Objectives: To retrospectively evaluate the accuracy of TT, its components and its modification; the Triple Test Score when the components are nonconcordant at Aga Khan University Hospital, Nairobi. Methods: Using ICD9 611.72 we identified patients with breast lump or mass in the period between January 2000 and July 2007. One hundred and thirty nine patients who had all the three components of TT with a complete histology report were recruited. Negative results for each component (Clinical Breast Examination, Mammography, and Fine Needle Aspiration Cytology) were awarded a score of 1 and positive results, each scored 2, 3 and 5 for CBE, mammogram and FNAC respectively. Results: Eighty three patients had non-concordant results. Forty two out of the 56 remaining were concordant for malignancy and 14 were concordant for benign disease. The sensitivities and specificities for the components were 83% & 72%, 53% & 72% and 88% & 75% for CBE, mammography and FNAC respectively. TT had sensitivity of 95.2% and specificity of 100%. A score of 8-10 (n=92) accurately predicted malignancy 94.4% of the time (Positive Predictive Value). A score of 5-7 (n=28) was only 64.3% accurate in predicting malignancy (Positive Predictive value). A score of 3-4 (n=19) predicted benign disease accurately 89.5% of the time (Negative Predictive Value). Conclusion: TT and its modification, the TTS can guide evaluation and management of palpable breast masses in AKUH. A TT result merits further evaluation with a tru-cut biopsy. A prospective study that validates the concept and further improves the results will be appropriate.

69. Al-Haqqah/The True Reality

I/We begin by the Blessed Name of Allah

The Immensely Merciful to all, The Infinitely Compassionate to everyone.

69:01
a. The True Reality!

69:02
a. What is the True Reality?

69:03
a. And what may enable you to comprehend the True Reality?
b. It is The Time of Resurrection and the Final Judgment.

69:04
a. The people of the tribes of Thamud and ‘Ad denied coming of the calamity: the Time of Resurrection and Final Judgment.

69:05
a. As for Thamud, they were destroyed by a severe thunderbolt.

69:06
a. And as for ‘Ad, they were destroyed by furiously violent cold blast of roaring wind storm -

69:07
a. HE unleashed upon them for seven long grueling nights and eight long days, in succession,
b. so that you could see the people lying dead on the ground like the uprooted hollow palm trunks.

69:08
a. So do you see any of them remaining now?
b. No!

69:09
a. And Pharaoh too, and those sinful communities before him, and the habitations of Sodom and Gomorrah overturned - which the people of Lot inhabited - indulged in grave sins.

69:10
a. When they denied and belied the Messenger of their Rabb - The Lord,
b. then HE seized them with an overwhelming seizing - one exceeding the other in its severity.

69:11
a. And when the water level rose high - exceeding all limits - during the forty days and forty nights of Great Flooding, WE carried you to safety in the floating Ark.

It has been the norm of the West that they allay the blessings and reforms brought into the world towards themselves as if the rest of the World was drenched into darknessof ignorance. The concept of Human Rights has laso been allayed to the British Magna Carta, though Islam has offered the most complete and comprehensive Human rights foundation hundred years earlier, aboout 600 years prior to Magna Carta. This research article pertains to a comparative study of the charter of Human Rights by UNO  and the comprehensive socio-political and economic sphere of Human Rights by Islam.

Intelligent Requirement Prioritization Using Fuzzy Logic

Software Engineering (SE) is one of the youngest engineering domains emerging and developing within past four decades or so. Still, a massive amount of research work has gone into shaping it the way we see it functioning today. As a result, we have an impressive knowledge repository to work with in the form of software development models, software engineering theories and practices etc. The aim of SE is to create software products, services or their artifacts in order to meet the requirements posed by stakeholders while meeting quality constraints imposed on them. In order to meet both these objectives, any software development derives its purpose and meaning from the requirements posed by various stakeholders. Requirement Prioritization is a very critical but often neglected area of requirement engineering. Experience has shown that without proper prioritization of requirements presented by various stakeholders, the end product usually fails to meet its objectives optimally. In fact in many instances, the product is considered a failure because it fails to meet its core objectives. Several requirement prioritization techniques have been presented by various researchers over the past years. Working with these techniques has exposed several limitations when applied in software projects. In this thesis, we have presented a novel multi-level value based intelligent requirement prioritization technique using fuzzy logic and as a facilitating process, we have redefined the “value” of software to better meet its objectives. We have introduced and applied the concept of requirement value to prioritize requirements. We have performed extensive experimentation using our proposed technique along with existing techniques. Results have shown that both our proposed definition and proposed technique have achieved superior prioritization results and consistency. The experiments have also shown that proposed technique is capable of delivering impressive prioritization under varying and often conflicting circumstances.
