Introduction: Breast conservation therapy compares to mastectomy in terms of overall survival and disease free survival. However it’s utilization in most parts of the sub-Sahara Africa is minimal with various factors cited: patient preference, provider preference and adverse effects of or limited access to radiation therapy. The aim of this study was to determine the clinico-pathological profile and outcomes of breast cancer patients treated with breast conservation therapy or mastectomy at a single referral centre. Methods: The Aga khan university hospital is one of the institutions in East Africa, where breast conservation therapy is practiced. We retrospectively analysed the details of breast cancer patients who underwent breast conservation therapy from the year 2008 to 2015 and compared this with patients who had undergone mastectomy during the same period. Results: Ninety one patients who had breast conservation therapy and 187 patients who underwent mastectomy were included in this study. The following factors were correlated with the type of surgery: age <35, 35-50, >51 years, parity, grade, stage, histological subtype, receptor status, neo-adjuvant therapy, adjuvant therapy and re-excision rate. The majority of patients, 38.1% (n=106) had stage II breast cancer. Although there was no stage to stage comparison between the breast conservation group and the mastectomy group, patients who had breast conservation were likely to be younger, with tumour grade I or III and luminal A molecular subtype. There was no significant correlation with being nulliparous, grade II tumours, having received neoadjuvant chemotherapy or re-excision rates. Patients who had mastectomy were likely to be older with grade II breast cancer and had adjuvant chemotherapy. Conclusion: The present study concluded that with proper pre-operative patient selection, breast conservation therapy has comparable clinico-pathological characteristics and outcomes to mastectomy.

اردو بازار، مال روڈ، انار کلی، ٹولینٹن مارکیٹ وغیرہ میں اکثر میرزا صاحب سے ملاقات ہو جاتی۔ میں ان کا بیگ اٹھا لیتا اور ان کے ساتھ ساتھ رہتا۔ گھر کی چھوٹی موٹی چیزیں وہ خود خریدتے تھے اور خوب بھاؤ تاؤ کرکے۔ ایک روز وہ شاپنگ وغیرہ کر چکے تو میں انھیں ویگن پر بٹھانے کے لیے انارکلی سٹاپ پر آیا۔ اس دن ویگنوں میں کچھ زیادہ رش تھا۔ میں نے کہا بھی، رکشے پر چلے جائیں مگر وہ حسبِ سابق نہیں مانے۔بڑی مشکل سے: پچیس(۵۲)نمبر ویگن میں انھیں جگہ ملی اور وہ بھی ڈرائیور کی پچھلی سیٹ(پھٹے) پر،دروازے کے بالکل ساتھ۔ انھیں بیٹھنے میں ذرا دشواری ہوتی تھی، بڑھاپے کی وجہ سے بھی اور ٹانگ میں خم کے سبب بھی۔ انھیں سوار ہونے میں قدرے تاخیر سی ہو گئی تو کنڈکٹر نے ان کا بازو سختی سے پکڑا اوریہ کہتے ہوئے ویگن میں گھسیٹ لیا: ”کیہہ بابا توں نخرے ای کری جانا ایں، جلدی کر!“اتنے میں ویگن چل پڑی اور میں وہاں کھڑا تادیر سوچتا رہا، اب: پچیس (۵۲)نمبر ویگن کے اس کندہ ناتراش کنڈکٹر کو کیا معلوم کہ یہ بابا کون ہے؟

Emergence of biomedical research and innovation with an unprecedented speed has created number of opportunities and challenges for policy makers. On the one hand, it is now possible to introduce tailor-made personal medication regime for an ailing patient to offer state of the art treatments. On the other hand, several ethical and legal issues have been raised due to the complex nature of emerging technologies.  Policy makers all over the world are constantly addressing these challenges by continuously upgrading their respective professional and regulatory frameworks. This article is an attempt to highlight Shariah maxims which have contemporary application in medical field. Lately, there has been a lot of interest in the debate of Shariah maxims and many scholars have used maxims-based analytical frameworks to show the dynamic application of Islamic law. This article builds upon those works by focusing on issues related to the medical field.

System Based Visiting Teachers Programme: Major Processes and Their Outcomes

Teacher education has traversed a long distance from off-site courses to school-based courses. All these courses were designed to develop teachers professionally, however, the approaches were different. The current trend of teacher education supports school-based professional development to a great extent. The System-Based Visiting Teachers Programme (SBVTP) was introduced by AKU-IED, as a variation of school-based teacher education programmes, to help teachers, to improve upon their classroom teaching practice. The study sets out to understand the major processes of SBVTP in which the visiting teachers (VTs) were engaged in during the programme, and their outcomes in terms of a change in VTs' beliefs and attitudes about teaching and learning. In this study four visiting teachers participated and shared their views about the different aspects of teaching and learning. The study was conducted in the qualitative paradigm. The data collection methods were document analysis; observations, interviews, and stimulation recall interviews with some informal talk with the research participants. The key findings of the study indicate that SBVTP was successful in bringing about a change in VTs' thinking about teaching and learning. However, there is no significant change in their practices mainly due to time constraints caused by their routine teaching responsibilities. In addition to these key findings, the study has also discovered some important contextual concerns that seem to inhibit the teachers in changing their practices. The study could help in understanding the factors, which facilitate bringing about change in VTs' practices.
