Background: The use of drugs to attenuate the haemodynamic response to laryngoscopy and endotracheal intubation is the standard of care during elective surgery. Current evidence is conflicting concerning the best agent and optimal dose for this purpose. In the majority of cases, Fentanyl is widely utilized to attenuate haemodynamic responses. Ketamine, an established available drug, has been scarcely studied in this regard at low doses and against varying doses of other common agents. Objective: The primary objective was to compare the overall occurrence of hypertension and tachycardia immediately pre-intubation (post-induction) until 10 minutes post intubation between the study group receiving fentanyl at 1.0 μg/kg and the other receiving Ketamine at 0.5 mg/kg. The secondary objectives were to compare the occurrence of post-induction hypotension and the occurrence of neuropsychiatric phenomena during emergence between the two groups. Primary outcome measure: Increase of Blood Pressure (systolic, mean arterial or diastolic) and Heart Rate >20% from the baseline. Secondary outcome measures: Decrease in Blood Pressure (post induction) >20% from baseline; Occurrence of neuropsychiatric manifestations. Study Setting: The Aga Khan University Hospital, Nairobi. Study Design: A prospective, double blind, superiority, randomized control trial. Sample size: A sample size of 108 participants, 54 in the Fentanyl arm and 54 in the Ketamine arm. Study population: ASA I and II patients aged 18-65 years scheduled for theatre for non-emergent surgery. Anaesthetic Procedure: 108 adult patients were randomly assigned to one of two groups corresponding to the drugs used for induction: (a) Fentanyl (1.0 μg/kg) added to Propofol (2.0 mg/kg) and Cisatracurium (0.2 mg/kg) (F, n = 54) and (b) Ketamine (0.5 mg/kg) added to Propofol (2.0 mg/kg) and Cisatracurium (0.2 mg/kg) (K, n = 54), and haemodynamic responses evaluated by determining heart rate and blood pressure immediately before laryngoscopy and at 2.5, 5, 7.5 and 10 minutes. This was in addition to standard of care. Data collection: Data was collected using a data collection tool (see appendix). Patients’ baseline characteristics, systolic blood pressure, mean arterial pressure, diastolic blood pressure and heart rate at baseline then at minute 1, 2.5, 5, 7.5 and 10 were recorded. Results: 108 ASA I and II patients scheduled to undergo elective surgery were included in this study, 54 participants (50%) in the Fentanyl arm and 54 (50%) in the Ketamine arm. خلیق احمد نظامی صاحب کے نام نامی سے میں علی گڑھ آنے سے پہلے ہی واقف ہوچکا تھا لیکن ملاقات ۱۹۶۸؁ء میں ہوئی جب راقم الحروف سرسید ہال کا طالب علم تھا اور وہ اس کے پرووسٹ مقرر ہوئے تھے۔
نظامی صاحب متوسط قد کے تھے لیکن ان کا جسم ایسا تھا کہ ہر لباس خواہ ہندوستانی ہو یا انگریزی ان پر خوب پھبتا تھا، ان کا رنگ گندمی تھا جس کی وجہ سے ان کے سفید بالوں کی جو قدرے لمبائی لیے ہوئے تھے سپیدی کا احساس بڑھ جاتا تھا۔ ان کی آنکھیں بڑی نہیں تھیں لیکن نہایت روشن تھیں جن میں ایک خاص چمک تھی، ان کے چہرے سے رعب عیاں ہوتا تھا لیکن خوف بالکل نہیں معلوم ہوتا تھا۔
نظامی صاحب مجسم علم تو تھے ہی لیکن ان کے سرتاپا عمل ہونے کا عقدہ اس وقت کھلا جب انہوں نے پہلی بار یونیورسٹی میں انتظامی عہدہ سنبھالا اور سرسید ہال میں پرووسٹ ہوکر آئے، ان کے زمانے میں ہال نے بڑی ترقی کی۔ میں اس کا عینی شاہد ہوں کہ وہ ہال کے دفتر میں حساب کا ایک ایک رجسٹر دیکھتے، مددگاروں کو انتظامات کی ایک ایک جزئی باتیں سمجھاتے انہیں راستہ بھی دکھاتے اور ایک ایک کام کی تاکید بھی کرتے اس کا نتیجہ یہ ہوا کہ ہال کے درد و دیوار سے لے کر اس کے سبزہ زار تک سب سرسبز اور شاداب ہوگئے۔ دراصل ہال کی...

Optimal Homotopy Asymptotic Method to Solution of Partial Differential Equations

Nonlinear Differential equations are of major importance in different fields of science and engineering. For complicated nonlinear problems exact solutions are not available and alternate way is to use numerical methods, Iterative methods or analytical techniques of perturbation. Numerical methods use discretization a have slow rate of convergence. Iterative methods are sensitive to initial conditions and in case of high nonlinearity they do not yield converged results. In perturbation methods small parameter is applied on the equation and hence cannot be applied for high nonlinear problems as they do not have small parameter. One of domain type methods is known as OHAM. This method is free from small parameter assumption and do not need the initial guess. The proposed method provides better accuracy at lower-order of approximations. Moreover the convergence domain can be easily adjusted. In this thesis OHAM is implemented for solution linear and nonlinear tenth order ODEs. Then its effectiveness and generalization is shown to a nonlinear family of PDEs, including Burger, Fisher, Burger’s–Huxley, Burger’s–Fisher, MEW and DGRLW equations. The results of the proposed method are compared with that of DTM, VIM, ADM, HAM and HPM, which reveal that OHAM is effective, simpler, easier and explicit. Apart from application to PDEs, OHAM is applied to couple system of PDEs. The coupled WBK, ALW, MB systems are used as test examples and results are compared with those obtained by HPM. OHAM is implemented to DDEs as well, and solution of MKdV lattice equation is presented for the illustration of proposed technique. The results are compared with HAM and HPM. In all cases the results obtained by OHAM are in close agreement with the exact solution and reveal high accuracy.
