Background: Head injury is a common traumatic condition seen in Kenya. Among the head injury patients seen at the Aga Khan University Hospital Nairobi, minor head injury patients are the highest proportion of non–fatal trauma patients. Minor head injury is described as witnessed loss on consciousness, definite amnesia or witnessed disorientation in a patient with a GCS score of 13–15 who has suffered a traumatic event. There has been considerable disagreement about the indication for a Computed Axial Tomography Scan of the Head (CT-head) in the large number of patients clinically classified as minor. The Canadian CT Head Rule was derived as a sensitive decision rule on the use of CT with the aim to standardize and improve the management of patients with minor head injury. The rule comprised of five high risk factors and two moderate risk factors(Appendix 1). At the Aga Khan University Hospital Nairobi, there are no protocols that guide the decision making by physicians on when to do a CT-head for patients who have suffered a minor head injury. According to a pilot study done, approximately 96% of patients with minor head injury have a CT–head done. Objective: To determine the change in proportion of CT-scan done in patients with minor head injury after introduction of the Canadian CT Head Rule Guide at the Aga Khan University Hospital Nairobi. Secondary objectives were to determine the proportion of patients with minor head injury and moderate risk factors according to the Canadian CT Head rule for whom CT was ordered, probability of neurosurgical intervention in patients with minor head injury and their outcomes on follow up. Study design: A Before - After study Method: A total of 84 eligible patients diagnosed with minor head injury were recruited at the Accident and Emergency Department. Forty - two patients were assessed and data on high risk factors and moderate risk factors of the CCHR, Glasgow coma scale, age, management plan and the Glasgow outcome score on follow-up collected with data collection form 1 (see appendix 2) in the 'before' group, thereafter the Canadian CT Head Rule (CCHR) was introduced and another forty-two patients were assessed according to data collection form 2 (see appendix 3). Results: The proportions of CT scans done in the 'before' and 'after' groups were 95.2% and 21.4% respectively. The difference of 73.8% between the two groups was statistically significant (CI 0.55 to 0.84).The proportion of patients

Pashtun Ulama have always been in the service of Islam. The origin of Pashtun people in Pakistan is the province of Khyber Pukhtunkhwa formerly known is North West Frontier Province (NWFP). Pashtun Ulama have served Islam on different fronts; some have done Jihad against the non-Muslims, some of them have offered their services in the field of Islamic education, ie, teaching in Madrassas, while some have served Islam via their writing commentary of Qur’ān, explanation of Ḥadīth and juristic problems. This article discusses the worthy contributions of one of the great commentators of Qur’ān Shaykh Ḥāfiẓ Muḥammad Idrīs, who belonged to Mardan in the Province of Khyber Pakhtunkhwa, He lived in the 19th century. The article encompasses a detailed biography of the writer and his worthy contribution in serving Qur’ān, Islam and Muslims focusing on his commentary on Qur’ān in Pashto language named as “Kashshāf Al Qur’ān”. The method used for the research is descriptive and qualitative. The review literature showed that such an esteemed and vigorous scientist of different sciences of Qur’ān and his valuable contributions are out of the reach of scholars and libraries that may lead to an irreparable loss of the Islamic legacy. The study came up with the outcomes of his efforts in the field exegesis of Qur’ān, and the way and pattern he followed in interpreting different meanings of the revealed literature. The study would help out Islamic scholars and will enrich the domain of the Qur’ān’s research with new thoughts and viewpoints

Exploring issue of Ecological Affordance in an English Class with Special Relevance to Localized English Discursive Practices

Exploring issue of Ecological Affordance in an English Class with Special Relevance to Localized English Discursive Practices The present study aims to determine the significance of localized English discursive practices with respect to all the basic language skills in an ESL class of elementary students at the Boys Campus, OPF Girls College, where the researcher has been a teacher for the past nine years. The researcher used Action Research for a period of nine months i.e. three terms. Therefore, 60 students were her research participants. This Action Research was based on mixed methods approach underpinned by the theoretical framework of Johnson's (2004) localized language learning theory. The research tools used were of both qualitative and quantitative nature. Among the qualitative techniques, the researcher used text analysis, stimulated recall pictures, stimulated recall interview, speaking test and target learners' creative work along with some quantitative techniques as a localized board game, the target learners' written tests as well as their filled-in questionnaire. The research objectives were first to determine the target learners' social world, second to measure their receptive skills with respect to foreign and local contexts and third to evaluate their productive skills in a localized context. Likewise, one of the research questions was how to localize an ESL class; the other two aimed at finding such text, language skill or activity that could have a stronger impact on the target learners. Data analysis was carried out using Johnson's (2004) dialogical model of SLA which states that language learning is a localized phenomenon. As for the contribution of this research, this study highlights the place of Localized Language Teaching (LLT) approach promoting Pakistani cultural and social values in ESL classrooms of Pakistani schools. It is concluded that localized English teaching methodology can have the stronger impact on ELLs' language learning abilities than a traditional one due to ecological affordances provided by ELLs' prior knowledge, their socio-cultural background and their central position in an English class.
