Background: Reliable distinction between AML and ALL is important for the selection of appropriate therapy. The final diagnosis of acute leukemia should be based on clinico-pathological, morphological, cytochemical and molecular cytogenetics data, together with immunophenotypic information. Flow cytometry has become the preferred method for the lineage assignment and maturational analysis of malignant cells in acute leukemias. Immunophenotyping has also allowed the detection of aberrant antigen co expression and the analysis of heterogeneity and clonality of malignant cells in acute leukemias. It is insufficient to merely make a diagnosis of acute leukemia, or even AML or ALL. Further classification is essential in order to determine the prognosis and select the most appropriate treatment. Furthermore, immunophenotypic characteristics of acute leukemias have not been described in Kenya yet and the existing immunophenotypic patterns of the disease in Nairobi are not known. Objective: The main objective was to determine the relative frequencies of Acute Leukemia immunophenotypes using commonly expressed markers and to describe clinicopathological characteristics. Setting: The study was carried out at Aga Khan University Hospital Laboratory between March 2009 and March 2010. Patients and methods: One hundred and thirty two consecutive blood and bone marrow specimens were analyzed for cytomorphological characteristics and immunophenotypic markers. The clinical-pathological characteristics were also recorded for each patient with acute leukemia. The immunological category was assigned using the EGIL criteria. The end point was the immunophenotypic profiles and the association of each immunophenotype with the clinical and pathological features. Immunophenotyping was done by Flow cytometry. Results: There were 132 patients in total in whom immunophenotyping was done. These comprised of 67(50.8%) males and 65(49.2%) females with M: F ratio of 1.03:1. The age ranged from 2 years to 87 years with a median age of 29.5 years. There were 88 cases of AML and 42 cases of ALL. Only 2 cases of biphenotypic leukemia were studied. AML was found to occur more in adults with a total of 76 (86.4%) cases in patients above 15 years of age and only 12 (13.6%) cases in those below 15 years of age. The commonest AML immunophenotype in all patients was AML-M2 accounting for 26 (29.5%) cases with 19 (21.5%) cases seen in adults and 7(7.95%) in children. لاہور کی ایک اطلاع سے یہ معلوم کرکے بہت افسوس اور دکھ ہواکہ حضرت الاستاذ مولانا رسول خاں صاحب بھی انتقال کرگئے۔ انتقال کے وقت عمر ایک سوچار یا پانچ کے لگ بھگ ہوگی۔ راقم الحروف کی طالبعلمی کے زمانہ میں اگرچہ دارالعلوم دیوبند کاہراستاداپنے فن میں ماہر اورکامل تھا لیکن چاراساتذہ ایسے تھے جو اپناجواب نہیں رکھتے تھے اوراربابِ علم کے حلقوں میں ان کی شہرت کاطوطی بولتا تھا ۔حدیث میں حضرتنا الاستاذ مولانا محمدانور شاہؒ الکشمیری ،ادب میں مولانا محمد اعزاز علی صاحبؒ ،منطق میں مولانا محمدابراہیم صاحب بلیاوی اورفلسفہ میں مولانا رسول خاں صاحب رحمہم اﷲ رحمۃ ً واسعۃً۔چنانچہ راقم نے جس سال منطق کی آخری کتابیں حمدؔاﷲ اورقاضی مولانا محمدابراہیم صاحب سے پڑھی تھیں اسی برس فلسفہ کی اعلیٰ کتابیں صدرا اورشمس بازغہ مولانا رسول خاں صاحب سے پڑھیں، یہ دونوں استاد کتاب نہیں بلکہ فن پڑھاتے تھے ۔طالب علم نے کیا اورکتنی عبارت پڑھیں ہے اس سے ان کوکوئی واسطہ نہیں ہوتا تھا۔طالب علم عبارت پڑھتے پڑھتے رک گیا یاانھوں نے ہی رکو ادیا تواب کتاب کودیکھے بغیر منہ اٹھا کر تقریر شروع کردی۔ اﷲ اکبر! یہ تقریر کیاتھی،معلوم ہوتا تھا کہ علم وفن کے سمندر میں طوفان اٹھ آیا ہے اور موجیں ہیں کہ ایک دوسرے سے ٹکرارہی ہیں۔اس تقریر میں نفس مسئلہ کی وضاحت ہوتی تھی اوراس کے بعد اختلافات کامِع دلائل بیان اور پھران پر تنقید وجرح اورمذہب حق کی ترجیح اوراس کے وجوہ۔ پھران دونوں حضرات کے درس کی ایک مشترکہ خصوصیت یہ بھی تھی کہ تقریر بڑے اطمینان اورسکون سے کرتے تھے۔ اس میں نہ عجلت پسندی ہوتی نہ گھبراہٹ اور نہ کہیں زورشور! البتہ فرق یہ تھاکہ مولانا محمدابراہیم صاحب بڑے شگفتہ مزاج، خوش تقریر اور بزلہ سنج بھی تھے ،اس لیے ڈبیاسے نکال کر پان کھاتے جاتے اورموقع موقع سے کچھ مزاحیہ...

One Unit and Sardar Abdul Rashid Khan

Sardar Abdur Rashid Khan, Chief Minister of NWFP remained in office from 23 April 1953 to 18 July 1955. He belonged to the police cadre of the province and was inspector general at the time of his appointment. He was not a politician and never thought of becoming the chief minister of the province. Nevertheless he performed his duties well. It was during Sardar Rashid period that One Unit scheme was adopted. It integrated all the provinces and other units of West Pakistan into a single province. The plan was initially supported by him. Later on, as promises made to him were not fulfilled, he developed differences with the central authorities for which he was removed from his post. ______

Antimicrobial Susceptibility and Esbl Prevalence of Clinical Pathogenic Pseudomonas Species

The current study was conducted from 2010 to 2014 in Khyber Teaching Hospital, Peshawar, Pakistan to determine the Susceptibility of Pseudomonas spp. to different chemotherapeutic agents and prevalence of extended spectrum β-lactamase. The samples were taken from three main tertiary care hospitals of Peshawar, Pakistan. During the study period, 3450 specimens including pus, urine, blood and burns etc. were collected and subjected to culture and sensitivity as per standard protocols. Samples were isolated and identified on the basis of standard biochemical techniques. Antimicrobial susceptibility was determined by modified Kirby-Bauer method and Minimum Inhibitory Concentrations were followed per the guidelines set by CLSI. The ratio of male to female under the study was 1:1.4. The most productive antimicrobial agent was class carbapenem (imipenem and meronem) against Pseudomonas spp. among the β-lactam agents whose susceptibility was 282 (84.43%) and 304 (91.02%) respectively. The resistance rate was highest for Tetracycline followed by Penicillin and the isolates were co-resistant to macrolides and flouroquinolones and a moderate activity was demonstrated to the Cephalosporins. A total of 232 isolates were recovered from hospitalized and 102 from OPD patients. Statistically significant values were obtained for 17 out of 20 antibiotics, as there is a remarkable variation in susceptibility pattern of OPD and Hospitalized isolates. In class carbapenems: imipenem and meronem showed 81.47%; 91.18% and 88.79%; 96.08% activity rate for indoor and outdoor strains respectively. Only tetracycline had a diminished rate for both in and out door (5.17% and 21.57%) isolates respectively. In-patient isolates showed higher rates of resistance to most tested antibiotics, compared with outpatient isolates. Overall, there was a moderate decrease in susceptibility rate of Pseudomonas spp. to the antibiotic analyzed over the last five years of the study. The MIC50 & 90 (μg/ml) of imipenem against Pseudomonas spp. was <1 and < 4, respectively. While results obtained by agar dilution method demonstrated the lowest MICs values with meropenem for Pseudomonas spp. MICs observed for carbapenems as compared to the other antimicrobials tested were higher. xix Initial screening and phenotypic confirmatory test for ESBL detection was carried out according to the CLSI protocols. Production of ESBLs was observed in 148 (44.32%) of the isolates and the remaining 186 (55.80%) were non-ESBL producers. Statistically, a significant difference was found in susceptibility of the Pseudomonas spp. to carpenems, quinolones and β-lactam/β-lacatamase inhibitors, among the ESBL producers. The resistance conferred by ESBL’s producing pseudomonas spp. to cephalosporin’s (CEC, CAZ, CRO and CFP) was 14.2%, 20.3%, 14.3% and 22.3% respectively, contrary to the non–ESBL’s, where they were comparably sensitive to 3rd and 4th generation cephalosporins. Treatment with third generation cephalosporin (ceftriaxone) is the only risk factor being associated with ESBL infections. β-lactamases of these strains analyzed genotypically by PCR with a series of primers specific for tem, shv and ctx-M genes. 100 samples were selected for PCR to detect tem, shv and ctx-M genes among the ESBL positive Pseudomonas spp. strains. A high proportion of isolates were confirmed for ctx-M gene which encodes a total of 48 strains followed by tem 38 and then 14 of them were shv genes.
